    The editorial by Stein et al. (2010) is timely and relevant given the development of DSM-V and the likely impact that such a development will have on mental health services in the USA. The revision of the DSM will also affect international psychiatric research and global practice thanks to the interplay between the development of DSM and ICD (Fulford & Sartorius, 2009). The editorial by Stein and colleagues is very rich and there are many themes suitable for further examination and discussion. For this response, however, we have chosen to focus on two themes: the use of the term ‘mental’ and the idea of psychiatric disorders being ‘in’ an individual

Depending on the context, agents exhibiting irrational behaviour may be described as foolish, ignorant, unwise or even insane.     Delusional beliefs and reason giving

    Delusions are often regarded as irrational beliefs, but their irrationality is not sufficient to explain what is pathological about them. In this paper we ask whether deluded subjects have the capacity to support the content of their delusions with reasons, that is, whether they can author their delusional states. The hypothesis that delusions are characterised by a failure of authorship, which is a dimension of self knowledge, deserves to be empirically tested because (a) it has the potential to account for the distinction between endorsing a delusion and endorsing a framework belief; (b) it contributes to a philosophical analysis of the relationship between rationality and self knowledge; and (c) it informs diagnosis and therapy in clinical psychiatry. However, authorship cannot provide a demarcation criterion between delusions and other irrational belief states

    THE EPISTEMIC INNOCENCE OF IRRATIONAL BELIEFS

    In an ideal world, our beliefs would satisfy norms of truth and rationality, as well as foster the acquisition, retention, and use of other relevant information. In reality, we have limited cognitive capacities and are subject to motivational biases on an everyday basis. We may also experience impairments in perception, memory, learning, and reasoning in the course of our lives. Such limitations and impairments give rise to distorted memory beliefs, confabulated explanations, and beliefs that are elaborated delusional, motivated delusional, or optimistically biased. In this book, Lisa Bortolotti argues that some irrational beliefs qualify as epistemically innocent, where, in some contexts, the adoption, maintenance, or reporting of the beliefs delivers significant epistemic benefits that could not be easily attained otherwise. Epistemic innocence does not imply that the epistemic benefits of the irrational belief outweigh its epistemic costs, yet it clarifies the relationship between the epistemic and psychological effects of irrational beliefs on agency. It is misleading to assume that epistemic rationality and psychological adaptiveness always go hand-in-hand, but also that there is a straight-forward trade-off between them. Rather, epistemic irrationality can lead to psychological adaptiveness, which in turn can support the attainment of epistemic goals. Recognising the circumstances in which irrational beliefs enhance or restore epistemic performance informs our mutual interactions and enables us to take measures to reduce their irrationality without undermining the conditions for epistemic success

    Delusions and Three Myths of Irrational Belief

    This chapter addresses the contribution that the delusion literature has made to the philosophy of belief. Three conclusions will be drawn: (1) a belief does not need to be epistemically rational to be used in the interpretation of behaviour; (2) a belief does not need to be epistemically rational to have significant psychological or epistemic benefits; (3) beliefs exhibiting the features of epistemic irrationality exemplified by delusions are not infrequent, and they are not an exception in a largely rational belief system. What we learn from the delusion literature is that there are complex relationships between rationality and interpretation, rationality and success, and rationality and knowledge
