Variação polimórfica em genes de proteínas de sistemas REDOX e biomarcadores circulantes associados envolvidos na etiopatogenia de tumores ginecológicos (cancro do colo do útero e leiomiomas uterinos)

Universidade de Lisboa, Faculdade de Ciências, 2013Os tumores ginecológicos são a maior causa de morbilidade e mortalidade nas mulheres em todo o mundo. A infecção pelo HPV é necessária, mas não suficiente, para o desenvolvimento do cancro do colo do útero. Para além do genótipo viral, é também importante a participação de outros factores no processo de infecção, transformação, imortalização celular e sua progressão. Os leiomiomas, por outro lado, desenvolvem-se a partir de uma única célula de músculo liso. São bem diferenciados e constituídos por uma deposição excessiva de colagénio e proteínas da matriz extracelular. No útero desenvolvem-se sob a influência local de factores de crescimento e hormonas sexuais. O papel do stresse oxidante em tumores tem sido associado à exposição de ROS que promove a carcinogénese e a sinalização metastática. Sabemos, também, que há uma associação entre o sistema renina-angiotensina e o stresse oxidante, tornando-se assim interessante estudar alguns dos seus componentes. Os estudos genéticos permitem associar os polimorfismos ao risco de desenvolvimento da doença em estudo. Os estudos bioquímicos permitem verificar se existem enzimas (biomarcadores) circulantes cujas alterações ao nível da sua actividade e concentração sejam reflexo de qualquer distúrbio no metabolismo e/ou mecanismos de acção que afectem o desenvolvimento da doença. Assim o principal objectivo deste trabalho incide no estudo da contribuição de polimorfismos dos genes CYBA, CAT, ECA 1 e Hp e dos biomarcadores circulantes EO e GSH no desenvolvimento de tumores ginecológicos benignos (leiomiomas) e malignos (cancro do colo do útero). Verificámos que no cancro do colo do útero, os polimorfismos CYBA C242T e CAT 262C/T podem estar associados a uma maior susceptibilidade a esta patologia. Existe ainda uma diferença significativa entre os níveis da actividade da EO e as concentrações plasmáticas de GSH das mulheres com patologia do colo do útero e das mulheres do grupo controlo. Em relação à população de mulheres com leiomiomas, o polimorfismo CYBA C242T pode estar associado a uma maior susceptibilidade de desenvolver leiomiomas. Existe também uma diferença significativa entre os níveis da actividade da EO das mulheres com leiomiomas e as mulheres controlo.Gynecological tumors are the leading cause of morbility and mortality in women worldwide. Moreover, leiomyomas develop from a single smooth muscle cell. They are well differentiated and constituted by an excessive deposition of collagen and extracellular matrix proteins. They can develop the uterus under the local influence of growth factors and sex hormones. Oxidative stress has been associated with tumors by the exposure to ROS, promoting carcinogenesis and metastatic signaling. It is also known that there is an association between the renin-angiotensin system and the oxidative stress, thus making it interesting to study some of its components. Genetic studies allow association studies between polymorphims and the risk of developing a disease. Biochemical studies are important in order to check whether there are enzymes whose current level changes in its activity and concentration may reflect any disorder in metabolism and/or action mechanisms affecting the development of the disease biomarkers. Thus the main objective of this work focuses on the study of the contribution of the polymorphisms from CYBA, CAT, ECA 1 and Hp genes and circulating biomarkers EO and GSH in the development of benign gynecologic tumors (leiomyomas) and malignant (cervix cancer). We found that CYBA C242T and CAT 262C/T polymorphisms may be associated with increased susceptibility for cervix cancer, and there is a significant difference between the levels of EO activity and GSH plasma concentration in women with cervix cancer and control women. We found that CYBA C242T may be associated with increased susceptibility for the development of leiomyomas. A importância da matriz extra-celular nas doenças associadas ao HPV
Trabalho Final do Curso de Mestrado Integrado em Medicina, Faculdade de Medicina, Universidade de Lisboa, 2021A matriz extra-celular (MEC) é descrita como o componente não celular dos vários tecidos constituintes do nosso organismo, é o elemento que mantém uma estrutura bioquímica capaz de manter a organização e arquitetura dos constituintes celulares, desses tecidos. Sabe-se hoje que representa uma entidade dinâmica e ativa do tecido vivo, responsável pela comunicação, adesão e proliferação celular. Por sua vez, a infeção por Papilomavírus humano (HPV) é a doença sexualmente transmissível mais comum e este vírus é um agente etiológico precursor de tumores epiteliais e glandulares. O subtipo 16 (HPV16), causador de infeções, nomeadamente na área genital, orofaríngea e anal é a principal causa de lesões pré-malignas e cancros nestes tecidos. Este trabalho vai incidir sobre a infeção causada pelo HPV, com o objetivo de compreender o papel da MEC e dos seus componentes na invasão, disseminação e patogénese dos tumores associados a este vírus. Foi feita uma revisão da literatura, com um total de 41 referências bibliográficas publicadas até Março de 2021, recolhidas através de 2 bases de dados (PubMed e Google Scholar). Efetivamente, o cancro deixou de ser considerado uma patologia explicada apenas pelo descontrolo da proliferação e apoptose, mas também pela desregulação do microambiente. O conhecimento aprofundado da dinâmica da MEC e da complexidade das suas vias de sinalização relacionadas com o comportamento maligno da célula são pontos fulcrais e muito promissores, nomeadamente no desenvolvimento de terapias dirigidas ao alvo.The extra-cellular matrix (ECM) is described as the non-cellular component of the various tissues of our organism, and it is the element that maintains a biochemical structure capable of maintaining the organization and architecture of the tissue constituents. It is known today that it represents a dynamic and active entity of the living tissue, responsible for communication, adhesion and cellular proliferation. In turn, human Papillomavirus infection (HPV) is the principle etiological agent of the most common sexually transmitted disease, and it is a virus that can cause precursor lesions of epithelial and glandular tumors. The type 16 (HPV16), which causes anal, oral, pharyngeal and anogenital infections, is the main cause of pre-malignant lesions and invasive cancers in these tissues. This work will focus on HPV infection, in order to understand the role of ECM and its components in invasion, spread and pathogenesis of tumors and its precursor lesions caused by that virus. A literature review was performed, with a total of 41 references published up to March 2021, collected through 2 databases (PubMed and Google Scholar). In fact, cancer is no longer considered a pathology explained only by uncontrolled proliferation and apoptosis, but also by the deregulation of the microenvironment. The in-depth knowledge of ECM dynamics and the complexity of its signaling pathways related to the malignant behavior of the cell are central and very promising points, namely in the development of new targeted therapies
O papel dos heparano-sulfatos nas infeções víricas
Trabalho Final do Curso de Mestrado Integrado em Medicina, Faculdade de Medicina, Universidade de Lisboa, 2023Os Proteoglicanos de Sulfato de Heparano (HSPG) encontram-se presentes tanto na membrana celular como na matriz extracelular (MEC), sendo libertados e mobilizados pela ação das metaloproteinases (MMPs) e da Heparanase (HPSE). Estes proteoglicanos caracterizam-se pela sua carga negativa (conferida pelo grupo Sulfato de Heparano – HS) que lhes permite o sequestro de água e catiões (sódio e cálcio) e também pela capacidade de se ligarem a uma grande variedade de moléculas (tais como, proteínas da matriz extracelular, fatores de crescimento, citocinas e quimiocinas, tendo também sido demonstrado que funcionam como correcetores celulares para um grande número de vírus) e assim regular diversas funções biológicas. Este trabalho tem como objetivo rever evidências do papel dos sulfatos de heparano nos mecanismos patogénicos das infeções víricas, com especial enfoque no vírus da hepatite C. Como exemplo baseámo-nos num estudo realizado no Laboratório de Genética da FMUL para tentar entender a influência de um polimorfismo funcional (rs4693608) do gene da HPSE na progressão para os estadios de fibrose, numa amostra populacional com hepatite C crónica (HCC). Identificou-se que a idade e as atividades séricas dos enzimas ALP, AST, ALT e GGT se encontravam mais elevadas e o número de plaquetas diminuído, nos estadios de fibrose mais elevados (F3/4). Na associação genótipo e fenótipo intermédio, houve uma relação deste polimorfismo com níveis mais elevados de IMC, GGT e RNA. Concluindo-se, assim, através dos resultados obtidos das associações entre o fenótipo intermédio e o fenótipo distante, e também do genótipo com o fenótipo intermédio, que este polimorfismo afeta a progressão da fibrose.Heparan sulfate proteoglycans (HSPG) are present both in the cell membrane and in the extracellular matrix (ECM), being released and mobilized by the action of metalloproteinases (MMPs) and heparanase (HPSE). This work aims to review evidence of the role of heparan sulfates in the pathogenic mechanisms of viral infections, with a special focus on the hepatitis C virus (HCV).We used a study conducted at the Laboratory of Genetics of FMUL as an example, which aims to understand the influence of the HPSE gene functional polymorphism (rs4693608) in the progression to fibrosis stages in chronic hepatitis C individuals. In the higher stages of fibrosis (F3/4), we observed that the age and the serum activities of hepatic enzymes (ALP, AST, ALT and GGT) were higher, and the number of platelets was lower. There was also an association between genotype and intermediate phenotype with the aforementioned polymorphism and higher levels of BMI, GGT and viral load (viral RNA). Our results provide evidence for an association between intermediate and distant phenotypes, and also between genotype and intermediate phenotype, leading us to conclude that the polymorphism investigated here affects the progression of fibrosis

The interplay between HPV, other STI and genital microbiome on the development of cervical cancer
Trabalho Final do Curso de Mestrado Integrado em Medicina, Faculdade de Medicina, Universidade de Lisboa, 2022Introdução: A importância da Microbiota Cervicovaginal na proteção de infeções (como o HPV) encontra-se bem estabelecida, nomeadamente através dos Lactobacillus spp., tal como o mecanismo através do qual o HPV conduz à Neoplasia Cervical. Contudo, não é possível classificar o HPV como um carcinogéneo completo. Desta forma, a importância de explorar a disbiose da MCV com a intenção de decifrar esta interação com o HPV, toma maior relevância.
Objectivos: 1) Comparação da composição do MCV de mulheres com e sem HPV e de mulheres com ASCUS e LSIL; 2) Caraterização das citocinas presentes no microambiente vaginal; 3) Avaliação dos índices do hemograma como biomarcadores de prognóstico; 4) Correlação entre MCV, genótipos de HPV e citocinas.
Metodologia: Trata-se de um estudo retrospetivo, observacional, multicêntrico e transversal. A análise da MCV realizou-se através do isolamento do ARN bacteriano 16s, e análise numa plataforma de NGS. As concentrações citocinas da MCV foram obtidas através de plataforma Multiplex. A análise estatística foi realizada em SPSS v 26.0. Um α de 0,05 foi considerado estatisticamente significativo.
Resultados: Realçando o cerne do estudo, verificou-se que MCV do tipo CST III e CST IV influenciam o surgimento de lesões cervicais. Observou-se que o INL impacta o prognóstico de ASCUS. Dentro da MCV, os Lactobacillus impedem o crescimento de espécies do CST IV, enquanto as últimas expressam relações simbióticas entre si e apresentam afinidade com genótipos específicos de HPV. Por fim, a RANTES eleva-se significativamente perante infeções cervicovaginais.
Conclusão: A importância da utilização do perfil de citocinas e da MCV é realçada na hipótese de prevenção do desenvolvimento de NC tal como na sua utilização como biomarcador de prognóstico. Within CVM, Lactobacillus prevent the growth of CST IV species, while the latter express symbiotic relationships with each other and show affinity for specific HPV serotypes. On the other hand, RANTES is significantly elevated in cervicovaginal infections. Universidade de Lisboa, Faculdade de Ciências, 2013Os tumores ginecológicos são a maior causa de morbilidade e mortalidade nas mulheres em todo o mundo. A infecção pelo HPV é necessária, mas não suficiente, para o desenvolvimento do cancro do colo do útero. Para além do genótipo viral, é também importante a participação de outros factores no processo de infecção, transformação, imortalização celular e sua progressão. Os leiomiomas, por outro lado, desenvolvem-se a partir de uma única célula de músculo liso. São bem diferenciados e constituídos por uma deposição excessiva de colagénio e proteínas da matriz extracelular. No útero desenvolvem-se sob a influência local de factores de crescimento e hormonas sexuais. O papel do stresse oxidante em tumores tem sido associado à exposição de ROS que promove a carcinogénese e a sinalização metastática. Sabemos, também, que há uma associação entre o sistema renina-angiotensina e o stresse oxidante, tornando-se assim interessante estudar alguns dos seus componentes. Os estudos genéticos permitem associar os polimorfismos ao risco de desenvolvimento da doença em estudo. Os estudos bioquímicos permitem verificar se existem enzimas (biomarcadores) circulantes cujas alterações ao nível da sua actividade e concentração sejam reflexo de qualquer distúrbio no metabolismo e/ou mecanismos de acção que afectem o desenvolvimento da doença. Assim o principal objectivo deste trabalho incide no estudo da contribuição de polimorfismos dos genes CYBA, CAT, ECA 1 e Hp e dos biomarcadores circulantes EO e GSH no desenvolvimento de tumores ginecológicos benignos (leiomiomas) e malignos (cancro do colo do útero). Verificámos que no cancro do colo do útero, os polimorfismos CYBA C242T e CAT 262C/T podem estar associados a uma maior susceptibilidade a esta patologia. Existe ainda uma diferença significativa entre os níveis da actividade da EO e as concentrações plasmáticas de GSH das mulheres com patologia do colo do útero e das mulheres do grupo controlo. Em relação à população de mulheres com leiomiomas, o polimorfismo CYBA C242T pode estar associado a uma maior susceptibilidade de desenvolver leiomiomas. Existe também uma diferença significativa entre os níveis da actividade da EO das mulheres com leiomiomas e as mulheres controlo.Gynecological tumors are the leading cause of morbility and mortality in women worldwide. HPV infection is a necessary but not sufficient cause for the development of cervix cancer. In addition to the viral genotype, the involvement of other factors is also important in the process of infection, transformation, cell immortalization and tumor progression. Moreover, leiomyomas develop from a single smooth muscle cell. They are well differentiated and constituted by an excessive deposition of collagen and extracellular matrix proteins. They can develop the uterus under the local influence of growth factors and sex hormones. Oxidative stress has been associated with tumors by the exposure to ROS, promoting carcinogenesis and metastatic signaling. It is also known that there is an association between the renin-angiotensin system and the oxidative stress, thus making it interesting to study some of its components. Genetic studies allow association studies between polymorphims and the risk of developing a disease. Biochemical studies are important in order to check whether there are enzymes whose current level changes in its activity and concentration may reflect any disorder in metabolism and/or action mechanisms affecting the development of the disease biomarkers. Thus the main objective of this work focuses on the study of the contribution of the polymorphisms from CYBA, CAT, ECA 1 and Hp genes and circulating biomarkers EO and GSH in the development of benign gynecologic tumors (leiomyomas) and malignant (cervix cancer). We found that CYBA C242T and CAT 262C/T polymorphisms may be associated with increased susceptibility for cervix cancer, and there is a significant difference between the levels of EO activity and GSH plasma concentration in women with cervix cancer and control women. We found that CYBA C242T may be associated with increased susceptibility for the development of leiomyomas. A importância da matriz extra-celular nas doenças associadas ao HPV
Trabalho Final do Curso de Mestrado Integrado em Medicina, Faculdade de Medicina, Universidade de Lisboa, 2021A matriz extra-celular (MEC) é descrita como o componente não celular dos vários tecidos constituintes do nosso organismo, é o elemento que mantém uma estrutura bioquímica capaz de manter a organização e arquitetura dos constituintes celulares, desses tecidos. Sabe-se hoje que representa uma entidade dinâmica e ativa do tecido vivo, responsável pela comunicação, adesão e proliferação celular. Por sua vez, a infeção por Papilomavírus humano (HPV) é a doença sexualmente transmissível mais comum e este vírus é um agente etiológico precursor de tumores epiteliais e glandulares. O subtipo 16 (HPV16), causador de infeções, nomeadamente na área genital, orofaríngea e anal é a principal causa de lesões pré-malignas e cancros nestes tecidos. Este trabalho vai incidir sobre a infeção causada pelo HPV, com o objetivo de compreender o papel da MEC e dos seus componentes na invasão, disseminação e patogénese dos tumores associados a este vírus. Foi feita uma revisão da literatura, com um total de 41 referências bibliográficas publicadas até Março de 2021, recolhidas através de 2 bases de dados (PubMed e Google Scholar). Efetivamente, o cancro deixou de ser considerado uma patologia explicada apenas pelo descontrolo da proliferação e apoptose, mas também pela desregulação do microambiente. O conhecimento aprofundado da dinâmica da MEC e da complexidade das suas vias de sinalização relacionadas com o comportamento maligno da célula são pontos fulcrais e muito promissores, nomeadamente no desenvolvimento de terapias dirigidas ao alvo.The extra-cellular matrix (ECM) is described as the non-cellular component of the various tissues of our organism, and it is the element that maintains a biochemical structure capable of maintaining the organization and architecture of the tissue constituents. It is known today that it represents a dynamic and active entity of the living tissue, responsible for communication, adhesion and cellular proliferation. In turn, human Papillomavirus infection (HPV) is the principle etiological agent of the most common sexually transmitted disease, and it is a virus that can cause precursor lesions of epithelial and glandular tumors. The type 16 (HPV16), which causes anal, oral, pharyngeal and anogenital infections, is the main cause of pre-malignant lesions and invasive cancers in these tissues. This work will focus on HPV infection, in order to understand the role of ECM and its components in invasion, spread and pathogenesis of tumors and its precursor lesions caused by that virus. A literature review was performed, with a total of 41 references published up to March 2021, collected through 2 databases (PubMed and Google Scholar). In fact, cancer is no longer considered a pathology explained only by uncontrolled proliferation and apoptosis, but also by the deregulation of the microenvironment. The in-depth knowledge of ECM dynamics and the complexity of its signaling pathways related to the malignant behavior of the cell are central and very promising points, namely in the development of new targeted therapies
O papel do sistema nervoso central no desenvolvimento de leiomiomas

O Sistema Nervoso Autónomo desempenha um papel crucial na homeostase corporal e está dividido em Sistema Nervoso Simpático e Sistema Nervoso Parassimpático. Foram correlacionados níveis elevados de noradrenalina, adrenalina e dopamina com o desenvolvimento tumoral. A variante do polimorfismo rs1042713 do gene ADRB2 faz com que os níveis de ação de noradrenalina e adrenalina se mantenham altos o que confere um fator de risco ao desenvolvimento tumoral.
O presente estudo foi realizado com o intuito de avaliar a contribuição do polimorfismo rs1042713 do gene ADRB2, de forma isolada ou em interação com os polimorfismos rs9282801 e rs2297518 do gene NOS2 no desenvolvimento de leiomiomas uterinos.
Recorreu-se ao software IBM SPSS Statistics para fazer estudos de associação entre os genótipos e a doença, tendo sido realizada uma análise isolada e outra em epistasia entre os polimorfismos.
Os resultados da análise isolada indicam que a variante do polimorfismo do gene ADRB2 é um fator de risco para o desenvolvimento de leiomiomas (OR=6,917 para o modelo dominante e OR=2,017 para o modelo recessivo).
Nas interações epistáticas, observou-se que o genótipo AA do rs1042713 do ADRB2, em interação com o genótipo CC do rs9282801 do NOS2 ou com o genótipo GG do rs2297518 do NOS2, aumentam o risco da doença (OR=3,1 e OR=3,636, respetivamente).
Este tipo de estudos permitem estabelecer relações de causalidade entre os polimorfismos em estudo, tanto isoladamente como em epistasia e o desenvolvimento de leiomiomas, que poderão ser úteis para o desenvolvimento de estratégias preventivas e de novos tratamentos da doença.Uterine leiomyomas are common benign tumors in reproductive-age individuals, due to their high hormonal dependence.
The Autonomic Nervous System plays a crucial role in body homeostasis and is divided into the Sympathetic Nervous System and the Parasympathetic Nervous System. Elevated levels of noradrenaline, adrenaline, and dopamine have been correlated with tumor development. The variant of the rs1042713 polymorphism in the ADRB2 gene leads to sustained high levels of noradrenaline and adrenaline, posing a risk factor for tumorigenesis.
This study was carried out to assess the contribution of the rs1042713 polymorphism in the ADRB2 gene, either alone or in interaction with the rs9282801 and rs2297518 polymorphisms in the NOS2 gene, to the development of uterine leiomyomas.
IBM SPSS Statistics software was used to conduct association studies between genotypes and the disease, with both independent and epistatic analyses of polymorphisms.
Results from independent analysis indicate that the ADRB2 gene polymorphism variant is a risk factor for leiomyoma development (OR=6.917 for the dominant model and OR=2.017 for the recessive model).
In epistatic interactions, it was observed that the AA genotype of rs1042713 in ADRB2, in interaction with the CC genotype of rs9282801 in NOS2 or the GG genotype of rs2297518 in NOS2, increases the disease risk (OR=3.1 and OR=3.636, respectively).
Such studies help to establish causal relationships between the polymorphisms under investigation, both independently and in epistasis, and the development of leiomyomas. This information could be valuable for developing preventive strategies and new treatments for the disease
