    Aims: The radial approach is safer than the femoral for percutaneous coronary procedures. However its feasibility is lower, mainly for technical issues, often related to failure to puncture or cannulate the radial artery. The ulnar approach is a valid alternative to radial. We aimed to test the incidence, feasibility and safety of a direct homolateral ulnar approach in case of failed radial sheath insertion. Methods and results: Five operators collected their 1-year activity (diagnostic and interventional) with focus on entry site. Entry site choice was left to operators' discretion. In case of failed radial sheath insertion, an attempt to cannulate the homolateral ulnar artery was mandated, if ulnar pulse was present. All patients in whom this attempt was performed were followed until discharge. Out of 2403 procedures (1271 interventions), the final successful entry site was radial in 66.5%, femoral in 31.0%, ulnar in 2.1% and brachial in 0.4%. Radial failure occurred in 117 patients (6.9%). In 75 patients, the radial failure was not due to sheath insertion (which was successful), but to lack of catheter support or to tortuosity of the sub-clavian/brachial arteries. In the remaining 42 (35.9% of all radial failures), a homolateral ulnar approach was attempted. A successful cannulation of the ulnar artery occurred in 36 patients (85.7%) with further performance of the complete procedure. Concerning local complications, 1 radial pseudo-aneurysm (treated with additional compression) occurred, while no cases of early hand ischemia were reported. Conclusions: In this multicenter registry, in case of failed radial sheath insertion, switching directly to the homolateral ulnar artery for percutaneous coronary procedures is feasible and it appears to be safe, without cases of symptomatic hand ischemia.     Feto-maternal relationships in goats during heat and cold exposure

    Maternal and fetal body temperatures were measured in five Boer goats, of mean mass 64 +/- 8 kg, using temperature-sensitive radiotelemeters implanted intra-abdominally. Body temperatures were recorded every 5 min. Throughout the last month of gestation, fetal temperature was approximately 0.6 (o)C higher than that of the mother, in normal laboratory conditions (ambient air temperature: 21-24 (o)C). This feto-maternal temperature difference between the goat fetus and its mother is similar to that found in other mammals, including sheep. When the pregnant goats were subjected to short-term heating and cooling, the difference between maternal and fetal body temperatures changed. Thus the mean difference between fetal and maternal body temperatures decreased from 0.4 to 0.2 (o)C during 2 h of heating, while it widened from 0.3 to 0.7 (o)C during 6 h of cooling. These data support the idea that the fetus is thermally protected from excursions of body temperature during changes in the mother's thermal environment. Reports of goat stock losses and abortions during cold spells in their natural habitats may be the result of more severe and/or prolonged cold exposures that not only adversely affect fetal or maternal body temperature, but also influence other aspects of metabolism.Peer reviewedfinal article publishe

    Over the last ten years, a significant part of the results in mathematics didactics has demonstrated the potential of reproductive problems to increase the mobility of the view on figures and to teach the notional matters in geometry. Some other recent researches suggest that, in this context, the language associated with the material practices of pupils should also be essential. The work I’m presenting here is part of this double approach and questions the potential of a square restoration situation and the role of oral language in the perpendicularity learning process in a 5th grade class. My analysis is based on the observation of the instrumented, visual and linguistic dimensions of the students acts confronted with this situation. Part of the consideration that the pupils' actions, verbal responses and interpreting are closely intertwined, this work then pays particular attention to the conditions of dimensional and instrumental deconstruction occurrence in a situation of figure reproduction and to the processes of personal and social construction of knowledge through the language.Depuis un peu plus de dix ans, une part importante des travaux en didactique des mathématiques a montré le potentiel des problèmes de reproduction pour favoriser la mobilité du regard sur les figures et travailler les contenus notionnels à enseigner en géométrie. D’autres recherches plus récentes avancent que, dans ce contexte, le langage associé aux pratiques matérielles des élèves joue un rôle essentiel. Le travail que je présente ici s’inscrit dans cette approche double et interroge le potentiel d’une situation de restauration d’un carré et le rôle du langage oral dans la construction du concept de la perpendicularité dans une classe de CE2 et de CM1. Mon analyse prend appui sur l’observation de la pratique des élèves confrontés à cette situation, et tout particulièrement ses dimensions matérielle, visuelle et langagière. Ce travail, qui s’inscrit dans la considération que les actions, les signes verbaux et la façon de penser des élèves sont étroitement liés porte alors une attention particulière aux conditions d’émergence des déconstructions dimensionnelle et instrumentale dans une situation de reproduction de figure ainsi qu’aux processus de construction personnelle et sociale des connaissances par le langage

    0015: Left ventricular rapid pacing: a new technique to simplify BAV and TAVI procedures

    AimsThe aim of this study is to demonstrate efficacy and safety of a new technique for rapid pacing during balloon aortic valvuloplasty (BAV) and Transaortic Valve Implantation (TAVI).This study first describes the method used safely and successfully for BAV and TAVI in a fragile adult population.Methods and results these technique consist in a left ventricular pacing through the 0.35 inch back up guidewire inserted into the left ventricule. The cathode of an external pacemaker was placed on the external end of the 0.35’ wire using an alligator clamp. The anode was placed (also using an alligator clamp) on a small needle piercing the subcutaneous tissue at the site of the anesthetized groin.The balloon or the TAVI catheter provides the necessary insulation.We performed consecutively 100 cases of BAV and TAVI. All cases were successfully conducted with a one for one pacing (160 to 200/mn). That’s how we obtain a significant blood pressure drop in all cases with a mean systolic pressure during stimulation of 45mmHg. The time procedure was 38mn for BAV and 65mn for TAVI. Only three patients underwent a venous temporary pacemaker at the end of procedure for conduction disturbances. All cases of TAVI (34) underwent a femoral venous catheter as a central catheter while the BAV population did not have a venous femoral puncture. No venous vascular complication or tamponade was observed in our population.ConclusionsThis study demonstrated that the rapid pacing trough the back-up left ventricle guidewire is equivalent to right ventricular pacing in term of efficacy.This method is safer due to the limitation of complications related to right ventricular pacing lead placement. Finally, it simplify the procedure by making it faster

    Rumen morphology and volatile fatty acid production in the Blue duiker (Cephalophus monticola) and the Red duiker (Cephalophus natalensis)

    Blue duikers and red duikers have very similar rumen anatomy. In both species the rumen is a simple, plesiomorphic, sacculated S-shaped tube, while the ostia are large and the ruminal pillars weak. These specialisations facilitate rapid digesta passage. The rumens are densely papillated, and the fungiform papillae are highly vascular to aid absorption. Many of the adaptations are characteristic of concentrate selectors. Total concentrations of numeral fatty acids are lower in the red duiker than the blue duiker. Propionic and butyric acid concentrations are higher in the blue duiker than the red duiker while acetic acid concentration are greater in red duikers. Caecal concentrations of volatile fatty acids (especially acetic acid) are greater than those in the rumen of the blue duiker, but likely contribute less energy to metabolism because of the small size of the caecum relative to that of the rumenfinal article publishe

    Cette thèse porte sur la description morpho-syntaxique du nââ numèè, langue de l’extrême-Sud de la Nouvelle-Calédonie, parlée par moins de 1 000 locuteurs dans les tribus de Touaourou et de Goro dans la commune de Yaté et de l’île Ouen dans la commune du Mont-Dore.Dans la première partie, l’auteur aborde les faits sociaux qui ont fait de la région du Grand Sud l’une des plus impactés par les guerres traditionnelles, la colonisation, l’évangélisation, la construction du barrage de Yaté et plus récemment, l’exploitation minière.Dans la deuxième partie, l’auteur procède à l’identification des phonèmes de la langue. Ensuite, il effectue une comparaison entre le système consonantique, vocalique et tonal ainsi établi et celui réalisé par Rivierre dans les années 70. Cette analyse d’écart lui permet de mettre en exergue les évolutions phonologiques subies par la langue en presque quarante ans. Pour définir les différents morphèmes de la langue, l’auteur utilise la méthode traditionnelle dite « distributionnelle ». Il propose ensuite une catégorisation des lexèmes.Dans la troisième partie, l’auteur traite des procédés morphologiques utilisaient en dérivation et en composition.Dans la quatrième partie consacrée à la syntaxe, l’auteur expose le syntagme nominal avec les différents modes et types de détermination. Cette partie comprend également un exposé des substituts du nom, des déictiques et du système numéral. Puis, l’auteur propose une description du syntagme verbal avec en point d’orgue, l’analyse des marqueurs aspecto-temporels et les constructions sérielles. Dans la partie dédiée aux circonstants, l’auteur s’intéresse au système d’orientation développé et les différents cadres référentiels utilisaient (absolus et relatifs).Dans la dernière partie, l’auteur expose les différents types d’énoncés (simples, complexes et marqués).This thesis deals with the morpho-syntactic description of Nââ Numèè, a language of the far south of New Caledonia, spoken by fewer than 1,000 speakers in the tribes of Touaourou and Goro in the town of Yate and the isle Ouen in the town of Mont-Dore.In the first part, the author explains the social facts which led to the great southern region being one of the most affected by traditional wars, colonization, evangelization, the construction of the dam at Yate and more recently, mining.In the second part, the author will identify the phonemes of the language. Then a comparison will be made between the consonant, vowel and tonal system thus established and made by Rivierre in the 1970s. This gap analysis allows the author to highlight the phonological changes in the language over almost forty years. The author then uses the traditional "distributional" method to define the different morphemes of language before proposing a categorization of lexemes.In the third part, the author deals with morphological processes used in derivation and in composition. In this chapter, it is interesting to see the increased use of verbal classifiers.In the fourth section dealing with syntax, the author offers a description of the nominal phrase with the modes and types of determination. This part also includes a presentation of the nominal substitutes, deictic and numeral systems. Then, the author describes the verbal phrase with special focus on the analysis of the aspect and temporal markers and serial verbs. In the part dedicated to the circonstants, the author focuses on the orientation system developed and the various repositories spaces used (absolute and relative).In the last part, the author exposes the different types of clauses (simple, complex and marked)
