Dimitrios Tsamis Karatasos : a symbol of Greek, Serbian and Bulgarian friendship
Two Serbian texts extol the contribution of Dimitrios Tsamis Karatasos to the Balkan joint effort to throw off the Ottoman yoke. These texts are analysed by the author within the historical framework of their period, so that the man’s personality and work may be accurately evaluated from a fresh viewpoint. More specifically, the author conducts a research on the tombstone of Dimitrios Tsamis Karatasos, which he discovered himself at Naoussa, and the octet engraved on it, which is also published here. HIV-1 DNA as a prognostic marker of immune reconstitution and clinical outcome in patients initiating antiretroviral therapy for the first time
Objective: This dissertation aims to evaluate the pre-cART HIV DNA burden as a predictive factor for immune reconstitution and clinical progression of the neurocognitive status of people living with HIV (PLWH). Methods: HIV DNA was isolated from peripheral blood mononuclear cells (PBMCs) and quantified using real-time PCR. Immune reconstitution was assessed over a period of up to four years. Piecewise-linear mixed models were used to describe changes in CD4 counts. The Montreal Cognitive Assessment (MOCA) scale was used to evaluate cognitive function. Results: In the immune reconstitution study, 148 PLWH were included. The highest rate of immune reconstitution was observed during the first trimester of therapy. There was a trend suggesting that higher HIV RNA levels were associated with a greater increase in CD4 counts, particularly during the first trimester of cART (difference above vs. below median: 15.1 cells/μL/month, 95% CI: -1.4 – 31.5, p = 0.073). Similarly, higher HIV DNA levels appeared to predict greater CD4 increases, particularly after the first trimester (difference above vs. below median: 1.2 cells/μL/month, 95% CI: -0.1 – 2.6, p = 0.071). The combination of high HIV DNA and RNA levels was significantly associated with a greater CD4 increase after the first trimester (difference high/high vs. low/low: 2.1 cells/μL/month, 95% CI: 0.3 – 4.0, p = 0.024). In multivariable analysis, lower baseline CD4 counts predicted a greater CD4 increase.In the cognitive assessment study, 116 virally suppressed PLWH participated (mean age: 47.6 years, 91.4% male). Of these, 60 individuals underwent re-evaluation after the COVID-19 pandemic, with a median follow-up of 3.1 years. The median MOCA score was 24 (22–26), with 35.3% of participants scoring within normal limits. A negative correlation was observed between MOCA scores and age (ρ = −0.283, p = 0.002), but not with CD4 count at diagnosis (ρ = 0.169, p = 0.071) or initial HIV RNA load (ρ = 0.02, p = 0.984). In the subgroup of participants with repeated assessments, MOCA scores were correlated with CD4 counts (ρ = 0.238, p = 0.069 in the first and ρ = 0.319, p = 0.014 in the second test). An improvement in performance was observed, with an increase in the median MOCA score from 24 to 25 (p = 0.02). Conclusions: In PLWH receiving successful treatment, pre-cART HIV DNA and HIV RNA levels serve as predictive markers of immune reconstitution. The MOCA scale can detect early changes in cognitive function in PLWH. Neurocognitive decline is associated with age and possibly CD4 levels at the time of testing, but not with baseline CD4 counts or HIV RNA or DNA levels.Σκοπός: Η παρούσα διατριβή αποσκοπεί στην αξιολόγηση του φορτίου του HIV DNA πριν την έναρξη της αντιρετροϊκής αγωγής ως προγνωστικού παράγοντα για την ανοσολογική αποκατάσταση αλλά και για την κλινική εξέλιξη της νευρογνωσιακής κατάστασης ατόμων που ζουν με τον HIV. Μέθοδος: Το HIV DNA απομονώθηκε από περιφερειακά μονοπύρηνα κύτταρα του αίματος (PBMCs) και ποσοτικοποιήθηκε με real-time PCR. Η ανοσολογική αποκατάσταση αξιολογήθηκε για διάστημα έως και τέσσερα χρόνια. Για την περιγραφή των μεταβολών των CD4 χρησιμοποιήθηκαν τμηματικά γραμμικά μεικτά μοντέλα (piecewise-linear mixed models). Η κλίμακα Montreal Cognitive Assessment (MOCA) χρησιμοποιήθηκε για την αξιολόγηση της γνωσιακής λειτουργίας. The Exemplary Life of Dimitrios Vikelas (1835-1908)
This paper describes the "satisfying curve" of Dimitrios Vikelas' life journey, starting from Syros in 1835, moving via Constantinople, Odessa, and Syros again, to London, Paris and finally Athens. It explores Vikelas' multiple aspects, as merchant, writer, traveller, lecturer and essayist, Olympic founder, educationalist, book collector and philanthropist, all of which were united in the public-spirited man of letters (logios). It sets Vikelas in the context of the Greek commercial diaspora, the world of the London expatriate Greek community, and the dynamic society of late nineteenth-century Athens, beginning in the 1870s to act as a magnet to Greek expatriates. The author stresses two qualities of Vikelas: his belief in the idea of a progressive Greek state marked by advances in education, culture, tourism and standards of public life; and the self-awareness and experience which inform his autobiographical writings, not only his memoir My Life but also his last such work, The War of 1897

Dimitrios Mavrokordatos Disproving the Hemostatic Myth of Aqua Binelli
“Aqua Binelli” or “Aqua Balsamica Arterialis” was a hemostatic compound invented by the Italian Fedele Binelli in 1797. Its hemostatic properties were viewed as the solution to treating hemorrhage in cases of wounds and surgical operations. Those who opposed the compound were persuaded of its total lack of effectiveness, supporting the view that hemostasis could be achieved by exerting pressure on, stitching, and ligating a vessel. The publications of Karl Ferdinand von Gräfe, Professor at the University of Berlin and fervent advocate of Aqua Binelli, helped spread the use of the compound in Europe. In 1832, however, one of his students, a young Greek named Dimitrios Mavrokordatos, who became the first Professor of Anatomy and Physiology in the newly established University of Athens in 1837, published in Würzburg his dissertation titled Observations on the Potency of Aqua Binelli (Weitere Nachrichten über das Binellische Wasser), where a completely different picture emerged relating to those applications of the compound that von Gräfe considered successful. The case of Mavrokordatos’ dissertation was a typical 19th-century scientific controversy between a young student and his teacher who enjoyed the support of his associates. Eventually, the myth of the hemostatic activity of Aqua Binelli was debunked. © The Author(s) 2020
