    The conclusion of the study by Lee et al.1 should be limited to about 60% of Koreans with normal acetaldehyde-metabolizing capacity from intact aldehyde dehydrogenase 2 (ALDH2). The nondrinker reference group appeared to be largely different from those of similar Western studies, potentially more prone to stroke, likely suggesting overestimation of beneficial effect by moderate alcohol intake.N

    Spontaneous Intracerebral Hemorrhage: Management

    Spontaneous non-traumatic intracerebral hemorrhage (ICH) remains a significant cause of mortality and morbidity throughout the world. To improve the devastating course of ICH, various clinical trials for medical and surgical interventions have been conducted in the last 10 years. Recent large-scale clinical trials have reported that early intensive blood pressure reduction can be a safe and feasible strategy for ICH, and have suggested a safe target range for systolic blood pressure. While new medical therapies associated with warfarin and non-vitamin K antagonist oral anticoagulants have been developed to treat ICH, recent trials have not been able to demonstrate the overall beneficial effects of surgical intervention on mortality and functional outcomes. However, some patients with ICH may benefit from surgical management in specific clinical contexts and/or at specific times. Furthermore, clinical trials for minimally invasive surgical evacuation methods are ongoing and may provide positive evidence. Upon understanding the current guidelines for the management of ICH, clinicians can administer appropriate treatment and attempt to improve the clinical outcome of ICH. The purpose of this review is to help in the decision-making of the medical and surgical management of ICH.Y

    Treatment of vascular dementia: Evidence from epidemiologic studies

    This article discusses the most relevant recent evidence on the prevention and treatment of vascular dementia from epidemiologic studies. Vascular dementia is the most common form of dementia after Alzheimer's disease and remains the only one that is preventable, although most of our knowledge about primary and secondary prevention of vascular dementia comes from direct extrapolation from works in stroke. Furthermore, at present there are no approved therapeutic agents for the treatment of vascular dementia. Recently, however, a number of reasonable studies on vascular dementia have been available. It is clear that rigorous control of vascular risk factors is important in primary and secondary prevention of vascular dementia, and perhaps in ameliorating its mild form. A rational therapeutic approach to the treatment of vascular dementia should be based on an understanding of its broad clinical spectrum and the diverse causes that may be responsive to currently available treatments.     Stroscope: Multi-Scale Visualization of Irregularly Measured Time-Series Data

    For irregularly measured time-series data, the measurement frequency or interval is as crucial information as measurements are. A well-known time-series visualization such as the line graph is good at showing an overall temporal pattern of change; however, it is not so effective in revealing the measurement frequency/interval while likely giving illusory confidence in values between measurements. In contrast, the bar graph is more effective in showing the frequency/interval, but less effective in showing an overall pattern than the line graph. We integrate the line graph and bar graph in a unified visualization model, called a ripple graph, to take the benefits of both of them with enhanced graphical integrity. Based on the ripple graph, we implemented an interactive time-series data visualization tool, called Stroscope, which facilitates multi-scale visualizations by providing users with a graphical widget to interactively control the integrated visualization model. We evaluated the visualization model (i.e., the ripple graph) through a controlled user study and Stroscope through long-term case studies with neurologists exploring large blood pressure measurement data of stroke patients. Results from our evaluations demonstrate that the ripple graph outperforms existing time-series visualizations, and that Stroscope has the efficacy and potential as an effective visual analysis tool for (irregularly) measured time-series data.N

    David G. Sherman Lecture Award: 15-Year Experience of the Nationwide Multicenter Stroke Registry in Korea

    The expected growth of stroke burden in Korea in early 2000s led to the initiation of a government-funded clinical research project with the goal of development and implementation of national stroke guidelines. The CRCS-K (Clinical Research Collaboration for Stroke in Korea) began as a part of this project. For stroke epidemiology and quality of care research, the CRCS-K developed a multicenter, prospective, stroke registry and began collection of data in 2008. Now, about 100 000 cases have been registered at 17 university hospitals or regional stroke centers and about 200 articles have been published based on the registry experience. The analysis of the 10-year secular trends showed overall improvement of stroke care and outcomes and areas for improvement. This large-scale, high-quality dataset provides opportunities to explore and compare treatment disparities using the comparative effectiveness research methods, design and conduct a registry-based randomized clinical trial, connect the registry data with other data sources including the national claims data and neuroimaging or genetic data, and collaborate with other international researchers. An international stroke registry consortium may be a viable future direction.N
