    Thrombocytopenic purpura during therapy with simvastatin

    The authors describe a case of very serious thrombocytopenic purpura occurring in a diabetic and hyperlipidemic woman during therapy with simvastatin, an HMG-CoA reductase inhibitor drug. Interruption of the medical treatment produced a prompt reversal of the thrombocytic attack: one year later the patient's platelet count remains within the normal range. The authors have not found other reports in the medical literature of similar serious reactions during therapy with an HMG-CoA reductase inhibiting substance; nevertheless, a causal nexus is hypothesized but not argued in this case, since further studies will be needed before this connection can be established

    The study undertaken for the Piano di manutenzione delle superfici di facciata del centro storico di Saluzzo has provided for an articulated and complex research phase which, through the critical survey of the building facades, the analysis of the materials and archival research, has made it possible to return the data acquired with graphic tables, tables and colour notation charts. In order to compose an exhaustive information framework of possible indications on materials and techniques for facade, stratigraphic analyses have been carried out, which also proved to be useful to integrate the data that emerged from bibliographic and historiographic sources. The historical archive of the city of Saluzzo has been an essential source of research, with the examination and study of documentary sources from the Modern Age documenting construction sites in the city. The research has made it possible to reconstruct the main renovations of the borgo di mezzo, in the lower part of the historical city, between the end of the XVIII century and the beginning of the XX century, providing original data on the maintenance of the historic building, the rebuilding of the fronts or the restoration of the facades

    Common bile duct involvement in chronic pancreatitis

    The features of the common bile duct (CBD) have been checked in 78 chronic pancreatitis patients in order to evaluate the frequency of extrahepatic bile duct changes, possible associated factors and effects on the outcome of the disease. Fifty of the 78 patients had an intrapancreatic stricture of the CBD and 24 of them also showed an upstream dilatation. No relationship was found between the features of the CBD and the severity of the pancreatitis, the presence of calcifications and the length of the disease. Humoral signs of impaired bile flow were found in 20 subjects, 19 of whom had an intra-pancreatic stricture of the CBD. Sixteen of these 19 patients also showed an upstream dilatation and five of them had overt jaundice. A surgical intervention on the biliary tree was carried out in 7 patients, all with a biliary stricture. Six of them also had a CBD dilatation over the stricture (p < 0.02 versus patients without CBD stricture). In conclusion CBD involvement during chronic pancreatitis is quite frequent but poorly predictable and should be checked in all patients with humoral cholestasis in order to prevent further complications

    Restoration work on a stately home (Ka’ D-Mezanis) in Rueglio, a settlement in the Valchiusella area between Ivrea and the southern part of the Valle d’Aosta, prompted a research project on rural and mountain architecture conducted in recent years by a research group from the Politecnico di Torino, under the scientific direction of Silvia Beltramo. The restoration works on Ka’ D-Mezanis were completed in 2018 and were carried out by applying a careful and conservative approach that made it possible to recover the constructive and decorative elements of the house. The works also offered the chance to reflect on the cultural heritage of the area, for both the local community and the public administration. It became clear that there was a need for an in-depth study of the historical tissue of the settlement, in order to appraise the feasibility of undertaking careful and sustainable renovation, in the light of the available economic resources and the architecture that was preserved. The research project covered the following main topics: a critical analysis of the building techniques and materials used at Ka’ D-Mezanis, to constitute a reference case study; a detailed survey of the masonry through stratigraphic analysis of the architecture; a study of other buildings with similar constructive and formal solutions in the territory of Valchiusella; and cataloguing of the historical urban landscape of the municipality of Rueglio by investigating functions and construction techniques with the definition of an abacus of masonry. The results of the ongoing research are presented to the scientific community as a starting point for shared reflections that can direct further development of the study and open up international collaborations

    The authors describe a clinical case with a peculiar sequence of unhealthy events. An operated by osteotomy woman presented a deep venous thrombosis of lever lower extremity with following pulmonary embolism. The patient was treated with heparin. After 5 days, the patient showed a thrombocytopenia, that was not determined by an immune mechanism. The heparin was stopped and the thrombocythemia returned to normal values. But the patient still presented somnolence, asthenia and hypotension. The tests of adrenocortical function showed below normal values. The abdominal CAT showed haemorrhagic necrosis of the suprarenal glands
