Prophylactic Total Gastrectomy: Techniques
No full textProphylactic total gastrectomy (PTG) represents the treatment of choice for carriers of pathogenetic CDH1 mutations in the context of hereditary diffuse gastric cancer (HDGC) syndrome. PTG consists in a total gastrectomy: it could be performed both through open or minimally invasive surgery depending on the surgeon’s expertise, but the higher rate of postoperative complications reported after laparoscopic procedures should be taken into account especially in the specific context of a prophylactic surgery. The intra-operative control of resection margins with frozen section is mandatory to exclude residual of gastric mucosa. D1/D1+ is suggested but D2 dissection can be considered. Roux-en-Y reconstruction is preferred. Long-term follow-up shows a good quality of life after this type of surgery despite the weight loss

relativamente al quinquennio 1991-95. Tali dati sono stati comparati con le serie storiche settantennali ed analizzati al
fine di interpretare l’evoluzione del regime degli afflussi-deflussi nel bacino del Tevere. L'analisi dei dati ha evidenziato una preoccupante tendenza alla riduzione dei deflussi del Tevere a Roma, più marcata nell’ultimo quinquennio (1991-95) con un valore di portata media pari a 179 mc/s rispetto a 235 mc/s rilevati nel periodo 1921-70
Post-pandemic scenarios of office workplace: new purposes of the physical spaces to enhance social and individual well-being
No full textThe hybrid working model, which allows employees to co-locate and work remotely (at home or elsewhere), seems to be prevailing, according to recent studies and the future projections developed in the post-pandemic new normality. The assessment of the two models has revived the debate over the new meanings of the office workplace and the experimentation on its spatial design.
This paper aims to identify the values and features that make the office space attractive and ensure its survival. It begins by analyzing the creative experience of remote work gained by workers during the lockdown, identifying both issues and benefits given by the acceleration in the use of advanced digital technology and the innovations in the management of remote work. Then, the study retraces the debate on the design of office spaces and services in recent decades and the processes that have accompanied it, involving more and more employees.
Besides a certain continuity detected between the issues addressed before and after the pandemic, the study emphasizes an emerging perspective that assigns a new centrality to the human being, understood in its complexity which goes beyond the user-centered approach, involving physical and mental well-being1. The paper will also present the sustainability perspective, specifically the awareness of the impact of human behaviours and design choices on an urban and global scale

No full textIn Bencivenga’s “Consequences in Kantian Ethics,” he offers a version of Kant’s ethics according to which the most rational approach to living one’s life is “to always imagine what might follow from one’s moves and to choose moves accordingly” (284), but according to which agents always nevertheless must be modest in their judgments about what they ought to do because the actual consequences of their actions might not turn out as they imagined. In this way, he tries to foreground the role of consequences in Kant's ethics. In this paper, I argue against Bencivenga and, in particular, against the idea that according to Kant, to determine whether an agent’s action is good we must wait for its consequences to unfold in time
