    Differential expression of fibronectin and its degradation products in malignant tumors

    This chapter discusses the differential expression of specific fibronectins (FNs) mRNA isoforms in association with malignant tumors, either in in vivo or in in vitro systems. FNs are high-molecular-weight multifunctional glycoproteins present in the extracellular matrices (ECM) of connective tissues playing a crucial role in cell adhesion, spreading, and migration. In conjunction with the loss of FN, transformed cells lose other ECM components, procollagens, laminin, and heparan sulfate as well as hyaluronic acid. FNs are particularly abundant in body fluids, loose connective tissue, granulation tissue, basement membranes, and in many developing tissues, but are often reduced or absent from the corresponding differentiated tissues. Since FN is maintaining the normal cell phenotype, the expression of FN probably represents a host reaction antagonizing tumor growth and invasion. The activation of proteolysis and the generation of FN degradation products with associated transformation enhancing factor activity may represent a relevant step in tumor growth and progression

    Purpose of reviewSchizophrenia Spectrum Disorders (SSD) are severe conditions that frequently produce significant impairment in cognitive performance, social skills and psychosocial functioning. As pharmacological treatment alone often provides only limited improvements on these outcomes, several psychosocial interventions are employed in psychiatric rehabilitation practice to improve of real-world outcomes of people living with SSD: the present review aims to provide a critical overview of these treatments, focusing on those that show consistent evidence of effectiveness.Recent findingsSeveral recent systematic reviews and meta-analyses have investigated in detail the acceptability, the effectiveness on several specific outcomes and moderators of response of different psychosocial interventions, and several individual studies have provided novel insight on their implementation and combination in rehabilitation practice.SummaryCognitive remediation, metacognitive training, social skills training, psychoeducation, family interventions, cognitive behavioral therapy, physical exercise and lifestyle interventions, supported employment and some other interventions can be fully considered as evidence-based treatments in SSD. Psychosocial interventions could be of particular usefulness in the context of early intervention services. Future research should focus on developing newer interventions, on better understanding the barriers and the facilitators of their implementation in clinical practice, and exploring the opportunities provided by novel technologies

    Schizophrenia during the COVID-19 pandemic

    PURPOSE OF REVIEW: The coronavirus disease 19 (COVID-19) pandemic is having a critical impact on healthcare systems across the world, as well as on mental health in the general population; however, evidence regarding the impact of the COVID-19 pandemic on people living with schizophrenia and on the onset of psychotic symptoms is currently emerging. RECENT FINDINGS: People living with schizophrenia are at an increased risk of COVID-19 and present worse COVID-19-related outcomes, including mortality. They show low levels of information and of concern regarding the possibility of contagion and infection but presented substantially stable levels of psychotic symptoms and even increased subjective well being during the pandemic. SARS-CoV-2, as well as the prolonged social isolation and the spread of misinformation, appear to be responsible in some cases for the onset of psychotic symptoms. SUMMARY: Clinicians should inform and educate their patients on the risks related to SARS-CoV-2 infection and COVID-19 and on the precautions that they should adopt to avoid contagion. Particular attention should be devoted to maintaining the continuity of care, especially in frail patients. Telemedicine might represent a valid support, but face-to-face visits in some cases remain essential. The hypothesis of a direct role of viral infection on the onset of psychotic disorders is currently debated, as viral involvement of central nervous system appears to be rather infrequent in COVID-19
