    The article, organised in the form of a discussion, aims to analyse the themes and motifs of Paolo Di Paolo’s works that are addressed to young audiences. The author has recently published a large number of volumes directed just at younger readers. When beholding the titles that constitute the author’s bibliography, the reader is struck by Di Paolo’s predisposition to transform literary classics: as much in Giacomo il signor bambino as in the edition of the Divina Commedia, the author’s goal is to try to reach his new readers. Therefore, Di Paolo proves to be a prolific author of the genre; in fact, in his bibliography, volumes of fairy tales with a classic slant, such as La mucca volante, are listed as well.L’articolo contiene un’analisi di temi e di motivi delle opere di Paolo Di Paolo dirette al pubblico più piccolo. L’autore negli ultimi anni ha pubblicato un cospicuo numero di volumi indirizzati, infatti, proprio ai lettori più giovani. Ciò che stupisce scorgendo i titoli che costituiscono la bibliografia dell’autore, è la predisposizione a trasformare i classici della letteratura: tanto in Giacomo il signor bambino quanto nell’edizione della Divina Commedia l’obiettivo è cercare di raggiungere i lettori più giovani.     L’articolo prende in esame – nel centenario della nascita – il “pensiero politico” di Pier Paolo Pasolini. Vengono pertanto scandagliati (soprattutto) i suoi scritti sulla società, ma frequenti sono le incursioni nella letteratura e soprattutto nel cinema di questo autore del tutto originale. Ne emerge lo spessore di concetti spesso usati dal poeta per analizzare e criticare il presente: Nuova Preistoria, nuovo capitalismo, consumismo, edonismo, il Potere, il Palazzo, il Processo, sviluppo e progresso, sacralità in primis. Si evidenzia inoltre come l’occhio e il giudizio di Pasolini siano stati principalmente guidati dalla passione e solo di rimando dalla ragione. Un ruolo non secondario nella sua percezione del reale ha inoltre assunto la sua omosessualità. Nella parte finale dello scritto si analizzano alcuni strumenti tecnici utilizzati dall’autore per svolgere la propria opera di comprensione del presente e della sua conseguente traduzione artistico-critica.The article examines - in the centenary of his birth - the "political thought" of Pier Paolo Pasolini. Therefore, it scrutinizes (above all) his writings on society, but there are frequent incursions into the literature and especially into the cinema of this completely original author. What emerges is the depth of concepts often used by the poet to analyze and criticize the present: New Prehistory, new capitalism, consumerism, hedonism, Power, the Palace, the Process, development and progress, sacredness in primis. It also shows how Pasolini's eye and judgment were mainly guided by passion and only by reason. His homosexuality also played a significant role in his perception of reality. In the final part of the paper we analyze some technical tools used by the author to carry out his work of understanding the present and its consequent artistic-critical translation

    Early migration of the cemented tibial component of unicompartmental knee arthroplasty: a radiostereometry study.

    Purpose: Unicompartmental knee arthroplasty has good clinical results but high revision rates. A unicompartmental knee arthroplasty design features an all-polyethylene and conforming tibial component, and we hypothesized that this may put at risk its fixation. Implant-to-bone micromotion was measured together with relevant clinical outcomes. Methods: The migration of the tibial component in twenty patients was measured at 3, 6, 12 and 24 months, using standard radiostereometry, along with the relevant clinical outcomes using the IKS scoring system. Results: The eighteen arthroplasties at 24 months were found successful, with very good functional (mean 87.7; SD 15.4) and knee scores (mean 94.8; SD 10.1). The means and the standard deviations of the maximum total point motion (MTPM) for the four follow-ups were, respectively, as small as 0.4 ± 0.1 mm, 0.6 ± 0.2 mm, 0.6 ± 0.3 mm and 0.7 ± 0.3 mm, an average over all patients but one. In this knee, these were 1.6, 2.1, 2.4 and 2.2 mm, therefore not at high risk of aseptic loosening according to the literature. Only one knee was found at this risk, having the MTPM from 12 to 24 months of 0.5 mm, and the component moving and sinking medially, and rising laterally. Conclusion: At 2-year follow-up, a successful implant-to-bone fixation can be achieved in conforming all-polyethylene cemented tibial component together with excellent clinical outcomes.     In vivo comparative efficacy of three surgical hand preparation agents in reducing bacterial count.

    Besides objective efficacy, the choice between an antiseptic-based liquid soap, or an alcohol-based hand rub for surgical hand preparation technique is based on personal preference. Glycerol is often added to the formulations in order to enhance tolerability; however, it has been recently reported as a factor reducing the sustained effect of surgical hand rubs. AIM: To compare the efficacies of three commercial products for hand decontamination. METHODS: The in vivo efficacy of an alcohol-based hand rub (isopropyl alcohol 40%; N-propyl alcohol 25%; glycerin 1.74%; triethanolamine salt of carbomer <1%) was compared with other widely used products in surgical hand antisepsis (chlorhexidine and povidone-iodine). All products were used according to the manufacturers' instructions. FINDINGS: The best results were achieved with the alcohol-based hand rub and these were sustained for a period of 3h. Some volunteers experienced skin peeling off the hands when using alcohol-based hand rub; in this group of participants, the bacterial count was reduced only by 0.91 ± 1.67 log10 compared with 2.86 ± 1.22 log10 in the group who did not show this phenomenon. CONCLUSION: Besides confirming the importance of alcohol-based hand rubs for surgical hand decontamination, the results suggest the value of assessing the characteristics, and response of healthcare workers' skin, that may contribute to the development of skin peeling, and the subsequent possibility of a paradoxical overcolonization of hands after surgical preparation with alcohol-based hand rub

    Learning from Galileo: Ventilator-associated Pneumonia Surveillance

    Ventilator-associated pneumonia (VAP) continues to be a major cause of morbidity and mortality and excess cost in critically ili patients. Traditional VAP surveillance, the key strategy needed to define the frequency of the phenomenon, and to gauge the success of prevention efforts, is complicated and subjective. Several studies have shown that the CDC's standard definition for VAP has a high interobserver variability, low sensitivity, and low specifìcity. Klompas and Klompas and colleagues have proposed a simpler definition of VAP; more-over, Klompas and colleagues have suggested shifting the focus of surveillance from VAP to ventilator-associated complications (VAC). Here we describe the results of the VAP-VAC surveillance among intensive care unit (ICU) patients of a teaching hospital in centrai Italy. Methods The study was conducted in a 900-bed teaching hospital in centrai Italy, in three medical-surgical ICUs from June to December 2011, including ali patients ventilated for more than 48 hours. For each study patient, a surveillance card was filled in with the following information: age, sex, days of hospitalization, and days of mechanical ventilation (MV). VAP and VAC were recorded, by physicians, according to the criteria proposed by Klompas and colleagues. VAC was defined as an increase in the patient's daily minimum positive end-expiratory pressure by 25 cm H20 sustained for at least 2 days or an increase in the daily minimum Fio, by at least 15 points sustained for at least 2 days after a minimum of 2 days of stable or decreasing daily minimum positive end-expiratory pressures and Fi0,s, respectively. The incidence of VAP and VAC was expressed as the number of first episodes per 1,000 ventilator-days. Moreover, the following outcomes were considered: duration of mechanical ventilation, ICU length of stay, and mortality. We compared conttnuous var-iable distributions using Wilcoxon rank-sum test and the categor-ical variable using the Fisher exact test The level of significance was set at P < 0.05. Results One hundred twenty-seven ICU patients with mechanical ventilation were monitored. The VAP rate was 1.32/1,000 MV-days (n = 2), and the VAC rate was 125/1,000 MV-days (n = 19). A signifìcant difference for duration of mechanical ventilation, hospital stay, and mortality was registered between VAC-positive and VAC-negative patients (P < 0.05), whereas VAP was a statistically signifìcant risk factor only for mortality. Dlscussion An incidence rate of 132/1000 MV-days was registered adopting Klompas's definition of VAP. The VAC definition identified a population of patients with longer ICU length stay, more days of MV, and higher mortality compared with patients without VAC These data are in agreement with Klompas's finding where the VAC definition was able to identify patients with increased mortality, duration of mechanical ventilation, and ICU length stay. Therefore, applying the VAC definition, it was possible to identify a population "at risk" whose complications were severe enough to require an increase in ventilator support. Our experience, although it has limitations due to the small number of patients, may highlight the feasibility and the useful-ness of the model, and above ali, the strategie importance of the identification of VAC-positive patients. In fact, it is likely that lowerìng VAC rate in an ICU could improve clinica! outcomes in terms of duration of hospital stay, duration of mechanical ventilation, and mortality. Moreover, VAC can be easily and quickly detected, and all the defining crìteria are objective. In conclusion, we would like to remember Galileo Galilei when he affirmed that is important "to measure what is measurable and make measurable what is not so", because, regarding infection prevention, we know that only what is effectively measurable makes a better outeom

    Ventilator Associated Pneumonia are the most common Infections of the Healthcare Organizations (IHO) in Intensive Care Units (ICU). Surveillance is essential for the control of IHO; to be effective, it must be based on an objective and accepted definition. The definition of VAP proposed by the Centers for Disease Control and Prevention (CDC) is complex and subjective. Recently, Klompas has proposed a definition of VAP based on a modification of CDC’s criteria, suggesting also to include the monitoring of the Ventilator Associated Complications (VAC), defined as an alteration of respiratory parameters FiO2 and PEEP. The aim of this study was to evaluate the applicability of a VAP/VAC surveillance program in the ICUs of a teaching hospital in Central Italy. The new surveillance strategy has been successfully implemented; the proposed monitoring tool has been shared and accepted. The inclusion of VAC in the surveillance system, was useful and has facilitated data collection thanks to an objective definition

    Results at a minimum follow-up of 5 years of a ligaments-compatible total ankle replacement design

    Background: A new design of 3-part ankle replacement was developed to achieve compatibility with the natural ligaments by allowing certain fibers to remain isometric during passive motion. Methods: We evaluate 75 ankle prostheses implanted from July 2003 to December 2008, at a mean follow-up 6.5. ±. 1.1. years (range 5-9 years). The mean age at surgery was 62. ±. 13. years (range 29-82). Results: The mean AOFAS scores achieved at pre-op and at last follow-up were respectively 37. ±. 5 (23-45) and 78. ±. 8 (64-98). (p. <. 0.001). Clinical range of motion of the ankle measured by goniometer pre op was 1°. ±. 2 of dorsiflexion and 12°. ±. 4° of plantarflexion; at last follow-up range of motion increased to 6°. ±. 5° in dorsiflexion (p. <. 0.01) and 18°. ±. 7° in plantarflexion (p. <. 0.05).Radiographs showed no loosening and little signs of radiolucency. Two revisions necessitated component removal, neither for implant loosening. The overall survival rate was 97.3%. Conclusions: Function and Range-of-motion showed significant improvements. These results demonstrate that ligaments-compatible shaped talar and tibial components, with a fully conforming interposed meniscal bearing, can provide satisfactory survival rates and clinical outcomes in the middle term
