Contributo dell’agobiopsia epatica nella risoluzione di alcuni problemi di Chirurgia d’Urgenza
L’agobiopsia epatica per via posteriore
The Authors present their experience in hepatic needle biopsies performed utilizing a posterior approach through the coronary ligament. this extraperitoneal approach avoids choleperitoneum and hemoperitoneum which may occur with the usual antero-lateral approach. the technique described is therefore safe and may be used even in patients with serious obstructive jaundice or with major coagulation deficiencie
La colangiografia per cutanea per via posteriore
Percutaneous transhepatic cholangiography is a very useful test in jaundice patients to determine chether biliary obstruction is extrahepatic or intrahepatic; furthermoreit is possible to identify the exact lesion site in order to suggest the surgical therapy.32 patients were studied with percutaneous cholangiography using a posterior approach. A success rate 94% without complications was obtained. Our results suggest that the posterior approach is preferable to the lateral one. In addition, the Authors compare Percutaneous posterior cholangiography to endoscopic retrograde cholangiography
Chirurgia delle metastasi epatiche da tumori dell’apparato urinario e genitale maschile
Hepatic metastases from genito-urinary tract neoplasms are infrequently eligible for surgical treatment. Nevertheless, some patients affected by primary renal adenocarcinoma or by Wilms' tumor, with single or lobe-confined hepatic metastasis, can be enrolled for a surgical excision of the hepatic lesion. Selection criteria of the patients are fully discussed; surgical treatment by anatomical resections (segmentectomy or hepatectomy) with primary parenchymal dissection are preferre
