Implement exercise in the oncological setting
Over the past 20 years, the understanding of the role of physical activity in cancer has been increased. Traditionally, patients were advised to rest, recovery, and save energy during and after anticancer treatments. Nevertheless, it is now clear that physical activity may help alleviate some side effects caused by therapies and a sedentary lifestyle; consequently, cancer patients should be encouraged to perform exercise. Epidemiological evidence shows that post-diagnosis physical activity is associated with enhancing patients ‘survival, especially in breast, colon, and prostate cancer. In cancer patients, exercise acts by improving health-related skills, particularly cardiorespiratory fitness, strength, and body composition. Moreover, several trials demonstrated that a regular exercise program effectively relieves some cancer and treatments ‘side effects, such as fatigue, nausea, and vomiting, thereby improving patients’ quality of life. The last update of the American College of Sports Medicine’ guidelines recommends that patients perform 90 minutes per week of aerobic exercise at moderate intensity, with strength activities twice a week. Despite these important benefits, in Italy, the spread of exercise-oncology programs and the research in the exercise oncology field are still poor, negatively impacting patients and producing a gap in the literature. The purpose of this thesis is trying to fill this gap, increasing the available literature, and proposing an exercise program based on patients’ needs and the current guidelines. Chapter one is dedicated to a brief introduction about physical activity in cancer. In chapters two, three, four, and five, the experimental studies that led to the development of patient-centred exercise program are presented. Chapters six and seven report two other studies investigating exercise as part of the multimodal approach in counteracting cancer cachexia. The last chapter is dedicated to a summary of the main thesis results

Anemia is one of the most common hematological manifestations occurring in cancer patients, related to a poorer prognosis. Among supportive care in cancer, exercise is emerging as a crucial tool demonstrating to reduce mortality risk, as well as to counteract several diseases and treatment-related adverse events. Physical Activity for Oncological Patients in COVID-19 Era: No Time to Relax
Whereas the coronavirus disease 2019 (COVID-19) storm is relentlessly progressing worldwide, a great effort from scientific societies has been made to give recommendations for safely continuing oncological care, prioritizing the interventions according to patients' condition and type and stage of tumor. Nevertheless, to date no specific suggestions regarding physical activity and exercise in cancer patients during the COVID-19 era have been released, neglecting the potential deleterious effects of quarantine and sedentary behaviour (imposed as containment measures against COVID-19), particularly in these subjects. Moreover, literature is constantly consolidating the crucial impact of regular physical activity in cancer in reducing recurrence and mortality risk. In this commentary, we discuss possible adaptations of the recently published exercise guidelines to the current pandemic emergency, proposing various modalities to prevent or mitigate the physical inactivity risk in cancer patients
Antibody-drug conjugates (ADCs) targeting TROP-2 in lung cancer
Introduction: The advent of antibody-drug conjugates (ADCs) represents a renewed strategy in the era of precision oncology. Several epithelial tumors harbor overexpression of the trophoblast cell-surface antigen 2 (TROP-2), which represents a predictor of poor prognosis and a promising target for anticancer therapy. Areas covered: In this review, we aim to collect the available preclinical and clinical data regarding anti-TROP-2 ADCs in lung cancer obtained through extensive literature research and screening of the available abstract/posters presented at recent meetings. Expert opinion: Anti-TROP-2 ADCs represent an innovative upcoming weapon against both non-small cell lung cancer and small cell lung cancer subtypes, pending the results of several ongoing trials. The proper combination and placement of this agent throughout the lung cancer treatment pathway, the identification of potentially predictive biomarkers of benefit, as well as the optimal management and impact of peculiar toxicity (i.e. interstitial lung disease) are the next questions to be answered
Development of Educational Print Materials for Physical Activity in Cancer: Evaluation of Readability and Suitability
Educational health materials may be important tools to increase physical activity in cancer patients. Nevertheless, most of the available resources regarding physical activity for cancer patients were found not suitable, had a low grade of readability, and thus, represent a significant barrier to behavior change. To date, little data about development criteria and evaluation of physical activity resources for cancer before their spread exist. The purposes of this study were (i) to describe the development of a physical activity guidebook designed for cancer patients and (ii) to test its readability and suitability. The guidebook was developed through multi-step passages, including group discussions, a literature review, identification of a motivational theory, and using previous research on exercise preferences, barriers, and facilitators to target the information. Two validated formulae were used to assess the readability, whereas thirty-four judges completed the Suitability of Assessment Materials questionnaire to evaluate the suitability of the guidebook. The guidebook was found readable for patients having at least a primary education, and the judges scored it as "superior" material. Our guidebook, following a rigorous method in the development phase, was considered to be suitable and readable. Further evaluations through clinical trials could investigate its effectiveness for behavior change and its impact on cancer patients
