    Living with a long-term, indwelling urinary catheter: catheter users' experience

    PURPOSE: The purpose of this study was to explore the experiences of long-term catheter users within a heterogeneous population.SUBJECTS AND SETTINGS: The sample comprised 27 community-dwelling long-term catheter users. Participants included 14 female users (4 urethral, 10 suprapubic catheter) and 13 male users (6 urethral, 7 suprapubic) between 22 and 96 years of age. Interviews were conducted in participants' homes except 1, which took place in a urology outpatient department based on the participant's preference.METHODS: A qualitative research design using an interpretive description approach was used for data collection and analysis. All interviews were electronically recorded and transcribed verbatim. Interpretive description involved familiarization with the data, thematic analysis, and the development of an interpretive account.RESULTS: The impact of the catheter and daily living adjustments that catheter users made are captured within 8 themes: (1) making adjustments; (2) managing away from home; (3) nighttime adjustment; (4) catheter problems; (5) social interaction; (6) support from others; (7) unpredictability; and (8) intimacy and body image.CONCLUSIONS: Catheter users' experiences of living with a catheter are shaped by a variety of interrelated factors. Some participants were determined to overcome catheter-related problems and develop self-reliance while others adopted a more resigned approach to living with a catheter. Having a catheter enabled some participants to experience greater freedom while others led more restricted lives as a consequence of catheterization

    Solutions to dilation equations

    THESIS 6323This thesis aims to explore part of the wonderful world of dilation equations. Dilation equations have a convoluted history, having reared their heads in various mathematical ?elds. One of the early appearances was in the construction of continuous but nowhere differentiable functions. More recently dilation equations have played a signi?cant role in the study of subdivision schemes and in the construction of wavelets. The intention here is to study dilatione quations as entities of interest in their own right, just as the similar subjects of differential and difference equations are often studied. It will often be Lp(R) properties we are interested in and we will often use Fourier Analysis as a tool. This is probably due to the author?s original introduction to dilation equations through wavelets. A short introduction to the subject of dilation equations is given in Chapter 1. The introduction is ?eeting, but references to further material are given in the conclusion. Chapter 2 considers the problem of ?nding all solutions of the equation which arises when the Fourier transform is applied to a dilation equation. Applying this result to the Haar dilation equation allows us ?rst to catalogue the L2(R) solutions of this equation and then to produce some nice operator results regarding shift and dilation operators. We then consider the same problem in Rn where, unfortunately, techniques using dilation equations are not as easy to apply. However, the operator results are retrieved using traditional multiplier techniques. In Chapter 3 we attempt to do some hands-on calculations using the results of Chapter 2. We discover a simple ?factorisation? of the solutions of the Haar dilation equation. Using this factorisation we produce many solutions of the Haar dilation equation. We then examine how all these results might be applied to the solutions of other dilation equations. A technique which I have not seen exploited elsewhere is developed in Chapter 4. This technique examines a left-hand or right-hand ?end? of a dilation equation. It is initially developed to search for re?nable characteristic functions and leads to a characterisation of re?nable functions which are constant on intervals of the form [n, n +1). This left-hand end method is then applied successfully to the problem of 2- and 3- re?nable functions and used to obtain bounds on smoothness and boundedness. Chapter 5 is a collection of smaller results regarding dilation equations. The relatively simple problem of polynomial solutions of dilation equations is covered, as are some methods for producing new solutions and equations from known solutions and equations. Results regarding when self-similar tiles can be of a simple form are also presented

    Friends For Men: A tutorial for developing Friends for men who face an unplanned pregnancy

    The moral and spiritual decay of the family in the United States is at a critical turning point. Christian men and women should be equally concerned about legislation that reconfigures the meaning of sexuality, family, and marriage. Regardless of a person's religious or moral beliefs, the institution of the family in the history of the U.S. has never been affronted by so many moral challenges. If the persistent attempts to redefine "life and the traditional family" are not stopped, society and the family will continue to decline. Despite challenges, the opportunity to make a difference and alter the overall course of our society is promising. A focused and dedicated effort is imperative for bringing change to fruition. Yet with approximately three thousand known-lives being aborted every day in the U.S. alone, the moral imperative of saving and protecting life remains a zenith problem. Under the promotional title of, "Friends for men...A tutorial for developing Friends for men who face an unplanned pregnancy", a series of formational training sessions were offered for men. The goal was to educate, train, develop and implement the missing link of training men as Friends at Birthmother Ministries. These men were trained to offer support and assistance to other men who are faced with an unplanned pregnancy. Small group interaction provided the opportunity to understand better transparency and vulnerability in personal relationships. This included enhancing skills necessary to communicate the physiological, psychological and spiritual ramifications of abortion for both men and women. Powerpoint presentations, role-playing and guided discussions added guidance for successful fathering and relationship building. A Friends packet for the Virginia and Maryland communities, pregnancy care centers and churches was presented. This packet also included life affirming adoption-option resources as well. At the end of the project, participants reported an improved understanding of the skills necessary for personal relationship building and guiding a man through his responsibilities in family dynamics. Those experiencing training became aware that young men faced with an unplanned pregnancy face a vast array of challenges and obligations. Unfortunately, today's father-absent generations have left young men empty-handed in facing their responsibilities.     Singing-Blocks: Considerations for a Virtual Reality Game to create chords and progressions

    Harmony and Chord progressions are the foundational architecture of a number of genres of Western music, from Classical, to Jazz, to popular music. For any musician, mastering the rules can be an important step in learning the skills of composition. The rules are difficult to learn, however. There have been efforts to create innovative educational software displays but to date Virtual Reality (VR) systems have not been investigated. The innately physical nature of VR creates an immersive experience that is impossible with other approaches. This work explains our concept of a VR educational game for young adult learners upwards of certain Western music genres. It draws subtle analogies to the architecture of harmony by presenting a number of levels in which the user builds chords and progressions. The aim is that they journey towards interacting creatively with the harmonics representation. Suggestions are also given as to how an effective evaluation procedure may be carried out in the future

    Audit of safety, efficacy, and cost-effectiveness of local anaesthetic cystodiathermy

    INTRODUCTION The aim of this study was to audit our experience of cystodiathermy under local anaesthetic (LA) at the time of flexible cystoscopy for recurrent superficial bladder transitional cell carcinoma (TCC). PATIENTS AND METHODS A total of 264 flexible cystoscopies were performed on patients with a past history of TCC. The number and site of recurrences were recorded and selected patients were offered cystodiathermy. Patient tolerability was noted. At follow-up, any recurrence was recorded. RESULTS Eighty patients (30%) had 91 procedures showing one or more recurrences. Fifty-one of the 80 patients (64%) were treated with cystodiathermy under LA. All completed treatment. Forty-five (88%) tolerated the procedure well. Forty-seven (92%) treatments were completed within 5 min. At a median follow-up of 15 weeks, 30 (59%) treated patients had no recurrence and three (6%) had recurrence at the site of treatment. CONCLUSIONS LA cystodiathermy is an effective and well-tolerated alternative to general anaesthetic cystodiathermy that enables treatment at the time of detection and may, thereby, reduce patient anxiety. </jats:sec
