Резюме. Метою даного дослідження було визначити ступінь розвитку набряку легень і рівень метаболізму сурфактанта у динаміці гострого ураження легень хлоридною кислотою. Досліди були проведені на 60 білих статевозрілих нелінійних щурах-самцях масою 200-220 г, яким моделювали гостре ураження легень шляхом інтратрахеального введення хлоридної кислоти при рН 1,2 в дозі 1,0 мл/кг на вдиху. Визначали індекс оксигенації рO2/FiO2 артеріальної крові, вміст білка у бронхоальвеолярному змиві, Wet/Dry індекси і коефіцієнт стабільності міхурців піни за R. Pattle.
Розвиток легеневого ушкодження за умови інтратрахеального введення хлоридної кислоти підтверджено як підвищенням втричі концентрації білка у БАЗ до кінця першої доби, так і розрахованими індексами розвитку легеневого набряку, які вказували на достовірне зростання набряку легеневої тканини і легеневого ексудату протягом часу спостереження. Встановлено прогресуюче зниження рівня показників поверхневої активності ССЛ протягом першої доби спостереження, що характеризується значним збільшенням рівня максимального поверхневого натягу з одночасним зниженням індексу стабільності на 45,8 % порівняно з контролем.
Отже, отримані дані чітко вказують на формування ГУЛ за рахунок розвитку набряку легеневої тканини та накопичення ексудативної рідини, і як наслідок, порушення оксигенації з розвитком гіпоксемії. Акумуляція рідини, багатої білками в альвеолах є чинником інактивації сурфактанту, що зумовлює зниженням індексу стабільності.

Summary. The aim of this study was to determine the degree of pulmonary edema and metabolic rate of surfactant in the dynamics of hydrochloric acid (HCl)-induced acute lung injury (ALI).
The development of lung damage in case of intratracheal introduction of HCl confirmed as three times higher concentration of protein in bronchoalveolar lavage after 24 hours of experiment, and rise calculated indices of pulmonary edema, which indicated a significant increase in lung tissue edema and pulmonary fluid in all groups of observation. It was found a progressive decrease in surface activity indicators of pulmonary surfactant system during the first day of observation: significant increase in the level of maximum surface tension with a simultaneous decrease in the stability index by 45.8%. Thus, the data indicate the formation of ALI by edema of the lung tissue and accumulation of exudative fluid, and as a consequence, impaired oxygenation with the development of hypoxemia. Accumulation of rich in proteins fluid in the alveoli is the factor of surfactant inactivation that leads to decreased of stability coefficient.

Hedging with Stochastic and Local Volatility
We derive the local volatility hedge ratios that are consistent with a stochastic instantaneous volatility and show that this ‘stochastic local volatility’ model is equivalent to the market model for implied volatilities. We also show that a common feature of all Markovian single factor stochastic volatility models, (log)normal mixture option pricing models and ‘sticky delta’ models is that they predict incorrect dynamics for implied volatility. As a result they over-hedge the Black-Scholes model in the presence of a market skew and this explains the poor delta hedging performance of these models reported in the literature. Whilst the traditional ‘sticky tree’ local volatility models do not possess this unfortunate property, they cannot be used for pricing without exogenous and ad hoc smoothing of results. However the stochastic local volatility framework allows one to extend a good pricing model into a good hedging model. Improved guidance is needed to optimise diagnostics and treatment of patients with thyroid cancer in Europe
Although thyroid cancer (TC) is generally associated with a favourable prognosis, there are certain high-risk groups with a clear unmet therapeutic need. Unravelling the genomic landscape of TC has recently led to the development of novel effective targeted treatments. To date, these treatments have mostly been evaluated in non-randomised single-arm phase II clinical trials and are consequently non-reimbursed in several countries. Furthermore, most of these agents must be tailored to individual patient molecular characteristics, a context known as personalised cancer medicine, necessitating a requirement for predictive molecular biomarker testing. Existing guidelines, both in Europe and internationally, entail mostly therapeutic rather than molecular testing recommendations. This may reflect ambiguity among experts due to lack of evidence and also practical barriers in availability of the preferred molecular somatic screening and/or targeted treatments. This article reviews existing European recommendations regarding advanced/metastatic TC management with a special focus on molecular testing, and compares findings with real-world practice based on a recent survey involving TC experts from 18 European countries. Significant disparities are highlighted between theory and practice related to variable access to infrastructure, therapies and expertise, together with the insufficient availability of multidisciplinary tumour boards. In particular, practitioners’ choice of what, how and when to test is shown to be influenced by the expertise of the available laboratory, the financing source and the existence of potential facilitators, such as clinical trial access. Overall, the need of a collaborative initiative among European stakeholders to develop standardised, accessible molecular genotyping approaches in TC is underscored.</p
