One health: professional stakeholder engagement key to tackling zoonotic disease
Globally, endemic and emergent disease risks persist as significant challenges to human and animal health. Rural livestock farming communities in low and middle-income countries are disproportionately affected by zoonoses and animal diseases due to poverty and concomitant poorly available health and veterinary services. Mansbridge and Dr. Vetja Haakuria explain why a One Health approach to tackling zoonosis is now broadly advocated and a successful strategy requires sector stakeholder recognition, inclusion, and engagement to ensure a holistic response to a complex problem
Equine Anthelmintic Resistance: Horse Owner and Yard Manager Perception of the Barriers Affecting Strategic Control Measures in England
Equine anthelmintic resistance poses a threat to the health and welfare of horses worldwide. With no new imminent anthelmintic classes, it is vital to decrease the resistance rate and preserve available classes. Traditional indiscriminate anthelmintic use is not synonymous with a strategic control ideology; however, many continue to implement outdated practices. In comparison to livestock farmers, there have been few social science studies examining horse owner behaviours. This study aimed to understand the perceived barriers faced by horse owners and yard managers to adopting a strategic approach to helminth control, and to explore their experiences. Semi-structured interviews permitted the exploration of the stakeholders’ beliefs, opinions, experiences, attitudes, and behaviours. Personal resources, internal influences, and external influences were identified by participants as the barriers to adoption of strategic controls. Two further themes impacting effective implementation of strategic controls emerged surrounding helminth information and source, as well as accurate dosing. Challenges associated with anthelmintic administration and bodyweight estimation suggest that inaccurate dosing is prevalent. Industry-wide implementation of a single ‘best practice’ could support prescriber decision-making to encourage uptake of effective strategic helminth controls. The barriers identified in this study could help influence guidance given to horse owners through a better understanding of their beliefs and behaviours
Value co-creation in high-involvement services: the animal healthcare sector
Purpose – The purpose of this paper is to investigate the significance of value co-creation to the UK animal healthcare sector from the perspective of the key industry stakeholders: clients, veterinarians and paraprofessionals. Design/methodology/approach – Value co-creation constructs in the sector were identified and measured using a mixed methods approach comprised of qualitative NVivo© thematic analysis of depth interviews (n ¼ 13) and quantitative exploratory factor analysis (EFA) (n ¼ 271). Findings – Qualitative results revealed nine underlying dimensions regarding service delivery in the sector: trustworthiness, communication, value for money, empathy, bespoke, integrated care, tangibles, accessibility and outcome driven service. EFA of professional survey data loaded onto seven latent factors, with strong value co-creation dimensions identified. Research limitations/implications – The sampling process is sufficiently representative and diverse to present meaningful and valuable results, however, surveying should be extended to include the client group. Due to the originality of the research replication of the study will be beneficial to the broader understanding and application of value co-creation to the high-involvement services of animal healthcare. Practical implications – Recognition of the importance of value co-creation to the sector should encourage professional stakeholders to develop and adopt integrated models of service provision and to provide improved levels of service quality. Originality/value – The paper makes an original contribution to knowledge regarding value co-creation in respect of high-involvement service provision. Its findings should be of value to academics interested in value co-creation in service sectors as well as animal healthcare practitioners seeking to offer better value and quality service provision
Sheep farmers’ attitudes towards lameness control: Qualitative exploration of factors affecting adoption of the lameness Five-Point Plan
In 2014, best-practice recommendations to treat and control lameness in sheep in the UK were consolidated into a national program, the Five-Point Plan (5PP). As recent evidence suggests that only the minority of sheep farmers are implementing all management practices listed in the 5PP, qualitative investigation is vital to ensure future promotion is aligned with psychological and contextual factors affecting farmers’ decision-making. This qualitative study sought to explore farmers’ attitudes and the factors affecting uptake of best-practice measures listed in the 5PP. Semi-structured interviews were conducted in 2019 with 12 sheep farmers from England and Wales. In accordance with Thematic Analysis and the principles of Grounded Theory, data collection and analysis were performed iteratively. Two overarching themes, delineated by subthemes, emerged during analysis; (1) Barriers to adoption of 5PP measures and (2) Motivation to adopt 5PP measures. Various farmer-centric factors and physical resources were identified as key barriers or obstacles that limited farmers’ ability to implement 5PP measures outright, or restricted their ability to make changes to facilitate future adoption. Conversely, internal and external influences were identified to increase farmers’ willingness and motivation to implement practices listed on the 5PP. Heterogeneity in farmer perceptions, attitudes, experiences and circumstances identified in this study highlights the difficulty in promoting a one-size-fits-all lameness control plan, where a unique combination of intrinsic factors, social influences, and physical restrictions affect implementation. Future initiatives should focus on removing barriers by changing farmers’ perceptions and mindset towards lameness control, and building farmers’ confidence in their ability to implement practices. Furthermore, farmers’ social licence to farm and their desire to improve their reputation within society, presents an important opportunity to further engage farmers in implementing control practices. Increasing peer-to-peer knowledge transfer opportunities and effective farmer-veterinarian communication and rapport could help establish 5PP measures as normativ
Co-Produced Care in Veterinary Services: A Qualitative Study of UK Stakeholders' Perspectives
Changes in client behaviour and expectations, and a dynamic business landscape, amplify the already complex nature of veterinary and animal health service provision. Drawing on prior experiences, veterinary clients increasingly pursue enhanced involvement in services and have expectations of relationship-centred care. Co-production as a conceptualisation of reciprocity in service provision is a fundamental offering in the services sector, including human medicine, yet the role of co-production in veterinary services has been minimally explored. Utilising a service satisfaction framework, semi-structured interviews (n = 13) were completed with three veterinary stakeholder groups, veterinarians, allied animal health practitioners, and veterinary clients. Interview transcript data were subject to the qualitative data analysis techniques, thematic analysis and grounded theory, to explore relationship-centred care and subsequently conceptualise co-production service for the sector. Six latent dimensions of service were emergent, defined as: empathy, bespoke care, professional integrity, value for money, confident relationships, and accessibility. The dimensions strongly advocate wider sector adoption of a co-produced service, and a contextualised co-production framework is presented. Pragmatic challenges associated with integration of active veterinary clients in a practitioner–client partnership are evident. However, adopting a people-centric approach to veterinary services and partnerships with clients can confer the advantages of improved client satisfaction, enhanced treatment adherence and outcomes, and business sustainability
Veterinary medicinal product regulation in sub-Saharan Africa: identifying barriers and opportunities for enhancing VMP regulatory systems
Effective regulation of veterinary medicines is essential to ensure veterinarians and animal keepers have access to assured quality, safe and effective products to prevent and treat animal disease. The maturity of national veterinary medicine regulatory systems varies between countries across sub–Saharan Africa and immature systems disincentivise manufacturers from bringing products to these markets. Common barriers to regulatory system strengthening identified by national regulatory agencies (NRAs) in the region include lack of financial resources, lack of trained personnel, and a need for suitable IT platforms to enable work-sharing and sharing of confidential data. Greater convergence and harmonisation of regulatory systems would enable more efficient use of resources through facilitation of regional NRA collaborations. Development of internationally agreed standards and guidance on good regulatory practice, a global regulators forum for exchange of best practice, and application of a self-assessment or audit tool, all of which exist for human medicines regulation, would enable NRAs for veterinary medicines to create and implement institutional development plans to achieve system strengthening. Independent assurance of NRA maturity would enhance opportunity for inter-agency reliance or unilateral recognition of regulatory decisions on product authorisation and good manufacturing practice inspection, currently under-utilised pragmatic approaches to ensuring necessary medicines are available quickly
Prevalence and Temporal Dynamics of White Line Disease in Sheep: An Exploratory Investigation into Disease Distribution and Associated Risk Factors
Lameness in sheep is a global health, welfare and economic concern. White line disease (WLD), also known as shelly hoof, is a prevalent, non-infectious cause of lameness, characterised by the breakdown of the white line. Little is known about the predisposing factors, nor the individual disease dynamics over time. Our exploratory study aimed to investigate the prevalence and temporal dynamics of WLD, and the associated risk factors. Feet of 400 ewes from four UK commercial sheep farms were inspected for WLD at four time points across 12 months. The change in WLD state at foot-level (develop or recover) was calculated for three transition periods. We present WLD to be widespread, affecting 46.8% of foot-level and 76.6% of sheep-level observations. States in WLD changed over time, with feet readily developing and recovering from WLD within the study period. The presence of WLD at foot-level, the number of feet affected at sheep-level and dynamics in development and recovery were driven by a variety of foot-, sheep- and farm-level factors. We provide key insight into the multifaceted aetiology of WLD and corroborate previous studies demonstrating its multifactorial nature. Our study highlights an opportunity to reduce WLD prevalence and informs hypotheses for future prospective studies
New insight into the prevalence and risk factors for three distinct hoof conformation traits in UK commercial sheep flocks
Lameness in sheep continues to be a global health, welfare and economic concern. Damaged, misshapen or overgrown feet have the potential to cause lameness either directly, or indirectly. There is a lack of understanding of the predisposing factors for different hoof conformation traits in sheep. Our exploratory study aimed to investigate the prevalence of, and risk factors for, three distinct hoof conformation traits relating to the sole and heel, hoof wall, and hoof wall overgrowth. Feet of 400 ewes from four UK commercial sheep farms were inspected at four time points across 12 months. For each conformation trait, a four-point ordinal system was used to score each individual claw, and foot-level scores were calculated. We present 92.4% of foot-level observations to be affected by ≥1 conformation traits. Whilst hoof conformation traits were correlated to some degree, a unique set of sheep-, foot- and farm-level factors were associated with each distinct conformation trait. We provide, for the first time, key insight into the multifaceted and multifactorial aetiology of hoof conformation in sheep, building upon previous landmark studies. Our results inform hypotheses for future prospective studies investigating the risk factors for adverse hoof conformation in sheep
Temporal trends in equine sperm progressive motility: a systematic review and meta-regression
In brief: Adverse trends in reproductive function are a concern in humans, companion, livestock, and wildlife species. This study indicates that equine populations are at risk of a comparable decline in sperm progressive motility. Abstract: There is increasing evidence reporting geographically sensitive adverse trends in human semen quality, with parallel trends observed in the dog sentinel. Despite significant economic and welfare complications associated with poor testicular function, trends in current equine populations are undetermined. Given the predictive value of sperm progressive motility (PMOT) in male factor infertility and fertilisation potential, research determining trends in this parameter is warranted. This research analysed trends in stallion sperm PMOT through systematic review and meta-regression. Using a comprehensive search strategy, Scopus, Embase (Ovid), Medline (Ovid), and VetMed (CAB direct) were scoped for eligible data. Using best practices, 230 meta-data points from 229 articles published from 1991 to 2021 were collated for meta-regression analysis. Sperm PMOT declined significantly between 1984 and 2019 (simple linear regression: b -0.340, P = 0.017; meta-regression: b -0.610, P ≤ 0.001). Overall and yearly PMOT declines were predicted at 33.51 and 0.96%, respectively (1984: 63.69 ± 5.07%; 2019: 42.35 ± 3.69%). Trends remained consistent irrespective of sensitivity analyses. Yearly and overall declines were stronger in western (yearly: 0.75%, overall: 26.29%) compared to non-western (yearly: 0.46%, overall: 10.65%) populations. Adverse trends contribute vital data to the debate surrounding declining semen quality, supporting the use of equines as novel comparative models for human reproduction. Results could have significant economic, health, and welfare consequences for equine breeding sectors. A comparable decline in human, dog, and horse sperm quality is indicative of a common environmental aetiology, indicating the need for a holistic One Health approach in determining causes and developing preventative strategies
Systematic Analysis of Breed, Methodological, and Geographical Impact on Equine Sperm Progressive Motility
Over the past five decades, there has been increasing evidence to indicate global declines in human semen quality. Parallel adverse trends measured in male animals indicate a potential environmental aetiology. This study evaluated the progressive motility (PM) of stallion ejaculate through a systematic review and meta-analysis. A total of 696 estimates of equine PM from 280 studies, which collected semen samples between the years 1990 and 2018, were collated for meta-analysis. The method of motility analysis, breed, season of collection, and geographical location were extracted. Simple linear regression determined temporal trends in stallion PM. Studies using microscopy estimated PM to be significantly greater compared to computer-automated methods (p ≤ 0.001). For Arabian breeds, PM was consistently higher than other breeds. Over time, there was a significant decline in PM for studies from Europe (n = 267) but a significant increase for studies from North America (n = 259). Temporal trends indicate the fertilising capacity of equine ejaculate has remained consistently high in the last three decades. That being so, variations observed suggest methodological, geographical, and individual stallion differences may significantly influence actual and reported stallion fertility potential
