    The aim of this study was to observe the clinical presentation and to descibe same clinical, endoscopic and prognosis aspects for outgoing patients with diverticulosis from Argeş area. Material and method. 192 patients were included in this study, with no prior medical records attesting the presence of diverticulosis. The study was conducted between 2011 and 2015 in ambulatory setting. Medical history, clinical exam and colonoscopy were undertaken during the visit. Inclusion criteria was the diagnosis of diverticulosis. Results. Colonoscopy was the method of investigation for confirmation of diverticulosis. Sex distribution was almost equal, for diverticulitis the sex ratio was raised towards female sex. Most patients were asymptomatic and from urban environment. Hemorrhoids were the cause for most of the hematochezia cases, only one case of diverticular bleeding. Discussions. the outgoing patients have mild symptoms and good clinical status compared to those needing hospitalisation. Differential diagnosis with irritable bowel syndrome needs a careful anamnesis. Conclusions. Frequent location of the diverticula was in the sigmoid and descending colon. Most patients have no symptoms and the number is slightly smaller compared with other studies, but they represent the majority in the analysed group. Most of the diverticulitis patients were older than 66 and females. There is an association between diverticular disease and irritable bowel syndrom

    Colonic Diverticulosis: From Asymptomatic Form to Major Complications

    Abstract Introduction This pathology is rare in patients younger than 40 years old (5%) and, in those older than 70 years old, almost 50% exhibit colonic diverticulosis. Most cases have mild symptoms and only 20% presents complications as: bleeding, bowel obstruction, abscess and secondary peritonitis. Aim. The aim is to analyse different types of complications in colonic diverticulosis and compare the data from literature to those in Argeș geographical area. Material and method. The medical records of 120 patients out of which 101 were admitted in gastroenterology department and 19 in general surgery department were analysed. Diagnosis methods: colonoscopy, computed tomography with contrast dye, simple abdominal radiograph for acute abdominal pain. Results. 40 patients (33.33%) with asymptomatic colonic diverticulosis, 28 patients (23.33%) moderate diverticulitis, 25 patients (20.83%) with moderate anorectal bleeding, 25 patients (20.83%) with secondary peritonitis, and 2 (1.68%) cases with complications. Conclusions. All the patients were older than 50 years old, 44 % of diverticulitis cases led to complications (almost half with bleeding and the rest with peritonitis). No bowel obstruction. In 99% of the cases, the localisation of the diverticula was at the level of sigmoid and descending colon. </jats:p

    Access to File: Rights of the Defence or Defence of the Rights?

    In November 2017, the Court of Justice of the European Union ruled in the Romanian Ispas case and decided that taxpayers are entitled to have access to file in VAT inspections. The unprecedented recognition of the fundamental right(s) of the defence leads to a number of questions as to the extent of the breach the Court made in the regular defence of national tax administrations. The paper aims to look into the lights and shadows of the European VAT inspections and to scientifically build a specific model for the appropriate exercise of taxpayers access to file, in particular with regard to VAT fraud cases. In this respect, the author shall consider comparative approaches and a thorough analysis of the Courts case law concerning VAT and procedural rights. Equally important, the paper shall consider the possible effect of the Ispas judgement on the general development of the European rights of defence in all tax cases
