Abstract
AIMS AND BACKGROUND:
In 2006 a survey was performed to define the resources available in Italy for the provision of radiotherapy services to head and neck cancer patients. This was the first initiative of the newly founded Head and Neck Group of the Italian Association for Radiation Oncology.
METHODS:
A questionnaire was sent to all 138 radiotherapy centers active in the country. Items investigated included total numbers of head and neck cancer patients treated per year, waiting time before the start of treatment, general technical issues, and integration with surgery and chemotherapy.
RESULTS:
Sixty-nine questionnaires were returned (50% response rate). The total number of patients treated was 4,670, averaging 68 cases per center. The larynx was the primary site most frequently involved. Average waiting time was 30 days and 47 days for nonresected and postoperative cases, respectively. The combination of chemotherapy and radiotherapy was delivered to nonresected and resected patients in 96% and 54% of centers, respectively. Survey response rates, waiting time, and the use of organ preservation protocols were the issues showing more variations across the country.
CONCLUSIONS:
This survey provides important data on radiotherapy resources available for head and neck cancer patients in Italy. The evidence of significant differences across the country concerning several relevant issues and the potential for cooperative clinical efforts in this relatively rare group of diseases urge the Group to plan further initiatives
Exhaled nitric oxide as a marker of lung involvement in Crohns disease.
Crohn’s Disease (CD) is an inflammatory bowel disease that is often associated with a variety of systemic manifestations, including airways involvement. The latter is in most cases subclinical, and requires expensive and invasive methods to be diagnosed. Fractional Exhaled Nitric Oxide (FENO) can be measured non invasively at low (bronchial air) and high (alveolar air) flows to reflect proximal airway inflammation, and systemic inflammation. The aim of our study was to compare both bronchial and alveolar FENO as an index of pulmonary involvement and of systemic inflammation in CD patients with at different stages of clinical activity and different treatments, with a group of healthy subjects studied. Thirty CD patients (age 43.10 ± 14.6 yrs) without clinical evidence of pulmonary diseases and 21 non smokers, not atopic healthy controls (age 35.1 ± 13.2 yrs) were enrolled. FENO was measured using a chemiluminescence analyzer at high and low flows. Bronchial (14.9 ± 10.2 ppb vs 10.1 ± 6.3 ppb, p=0.049) and alveolar FENO (4.4 ± 2.2 ppb vs 2.6 ± 1.9; p=0.006) were significantly higher in Crohn’s disease than in healthy controls, respectively, and bronchial FENO was significantly higher in CD patients treated with immunomodulators (20.4 + 11.5 ppb) than in those treated with mesalazine (7.9 + 4.2 ppb, p=0.04). Both bronchial (p=0.0016) and alveolar FENO (p=0.017) were positively correlated with Crohn’s Disease Activity Index. In conclusion, our results for bronchial FENO confirm subclinical pulmonary involvement in Crohn’s disease. Alveolar FENO may be of clinical value during follow-up of these patients as a surrogate marker of systemic inflammatio
Degree of malignancy of maxillary sinus cylindromas in relation to histologic characteristics
The authors, by analysing 31 cylindromas and 65 squamous cell carcinomas of the maxillary sinus, reached the conclusion that at a T2–T3, identical stage of development and with similar treatment, the carcinoma has a worse prognosis than the cylindroma, the latter being influenced in its clinical evolution by its histologic characteristics. In particular, the solid cellular pattern negatively affects the survival period. </jats:p
Video-Assisted Cleft Palate Surgery: Preclinical Comparison Between Endoscope- and Exoscope-Based Approaches
Objective: Using both endoscope and exoscope in cleft soft palate surgery is not widespread, despite the potential advantages related to view magnification, ergonomic posture of the surgeon, and involvement of the surgical team.Design: The aim of the current study is to compare endoscopic (Olympus Visera((C))) and exoscopic (Karl-Storz Vitom((C))) assistance in cleft soft palate surgery in a preclinical cadaver setting.Setting: A formalin fixed specimen was dissected to mimic the anatomical conditions of a cleft soft palate.Participants: Ten young surgeons with limited experience in transoral surgery were involved in the exercitation on the specimen. Interventions: The exercitation consisted of 4 tasks: (1) device setting; (2) identification of muscle plane; (3) muscle suturing; (4) oral mucosa suturing.Main outcomes: Participants were timed while performing each task both with exoscope and endoscope and asked to fill in 2 questionnaires related to the visual systems used (NASA Task Load System TLS and VAS 1 -10).Results: All surgeons completed the 4 tasks with both the endoscope and exoscope. The execution times were similar except for faster setting of the exoscope. Participants felt that completing surgical exercises using the exoscope required less physical, intellectual, and temporal efforts compared to the endoscope. The exoscope was also more appreciated for its handling, 3D visualization, and limited encumbrance.Conclusions: Exoscope scored better both at NASA TLS and VAS 1 -10 and required a faster setting than endoscope. Further clinical in-vivo studies are required to explore the advantages of these devices in cleft palate repair

LAUREA MAGISTRALENella presente tesi sono state riportate le considerazioni che hanno portato alla riprogettazione del sistema di ricircolo dell’aria presente all’interno dei frigoriferi Candy, così da indurre proprietà antibatteriche.
Partendo da una macroanalisi dei contaminanti presenti nell’aria e delle tecnologie esistenti per la loro filtrazione e abbattimento si arriva alla analisi dettagliata dei batteri, virus e altri microorganismi che possono giungere all’interno del corpo umano attraverso l’alimentazione; causando malattie di origine alimentare e infezioni virali.
La progettazione del dispositivo viene eseguita successivamente all’analisi dei fondamenti e dei sistemi basati sulla biossido di titanio usato per la degradazione fotocatalitica di una serie di contaminanti in fase gassosa e liquida.
Ulteriori analisi sono state svolte sulla possibilità di attivare tale processo attraverso la tecnologia dei Light Emitting Diode (LED) a emissione ultravioletta in grado di emettere specifiche lunghezze d’onda necessarie per il pieno sfruttamento delle proprietà del biossido di titanio.
Il dispositivo è stato successivamente prototipato ed utilizzato per la realizzazione delle prove di abbattimento batterico, i cui risultati sono stati riportati all’interno della presente relazione
