    Characterization of Laryngopharyngeal Reflux in the Elderly Population.

    INTRODUCTION: Laryngopharyngeal reflux (LPR) often is not diagnosed due to its presentation without heartburn. The aim of this study was to analyze the signs and symptoms of elderly patients undergoing treatment for LPR using subjective as well as objective measures including strobovideolaryngoscopies, 24-hour pH studies, and manometry to highlight appropriate considerations for diagnosis and treatment. METHODS: This was a retrospective review of patients with laryngopharyngeal reflux who were seen from January 2010 to February 2021 in the practice of the senior author (Robert T. Sataloff) who had undergone 24-hour pH testing, and for whom all data were available. Patients aged 60 years and older were considered elderly and compared to patients younger than 40 and between the ages of 40 and 59. However, patients in their 60s, 70s, and 80s were evaluated and compared separately. Objective reflux findings, 24-hour pH studies, and manometry were compared between groups. RESULTS: Two hundred and eighty-eight patients being treated for LPR were included, 100 of whom were over the age of 60. While 94.1% of patients younger than 40 had a chief complaint of hoarseness, 71% percent of those aged 60 and older complained of hoarseness. Patients aged 60 and older were more likely to present with no voice complaint as their age increased (P = 0.001). Lower esophageal sphincter residual pressure was 12.15 ± 6.28 in elderly patients compared to 4.90 ± 3.38 in patients younger than 40 (P \u3c 0.001). Upper esophageal sphincter (UES) basal pressure was decreased in elderly patients at 83.84 ± 50.53 compared to 105.92 ± 51.49 in patients younger than 60 or younger (P = 0.029). Older patients in their 70s and 80s also had lower UES basal pressures at 75.79 ± 47.66 and 63.45 ± 14.50 (P = 0.003). Additionally, while 71.4% of patients younger than 40 had normal esophageal motility (Chicago classification v4), only 40% of elderly patients had normal esophageal motility (P \u3c 0.001). More severe ventricular obliteration also was present in elderly patients compared to non-elderly patients (P = 0.032), and thick endolaryngeal mucus was less likely to be present in elderly patients (P = 0.007). Reflux finding score differed between age groups with patients aged less than 40 scoring 14.78 ± 2.5, 40-59 year olds scoring 15.83 ± 2.13 and patients 60 and older scoring 15.48 ± 2.43 (P = 0.018). CONCLUSION: Elderly patients diagnosed with LPR have different presentations of the disease compared with younger patients

    Overweight, obesity, and cardiovascular disease in heterozygous familial hypercholesterolaemia: the EAS FH Studies Collaboration registry

    Background and Overweight and obesity are modifiable risk factors for atherosclerotic cardiovascular disease (ASCVD) in the general popuAims lation, but their prevalence in individuals with heterozygous familial hypercholesterolaemia (HeFH) and whether they confer additional risk of ASCVD independent of LDL cholesterol (LDL-C) remains unclear. Methods Cross-sectional analysis was conducted in 35 540 patients with HeFH across 50 countries, in the EAS FH Studies Collaboration registry. Prevalence of World Health Organization–defined body mass index categories was investigated in adults (n = 29 265) and children/adolescents (n = 6275); and their association with prevalent ASCVD. Results Globally, 52% of adults and 27% of children with HeFH were overweight or obese, with the highest prevalence noted in Northern Africa/Western Asia. A higher overweight/obesity prevalence was found in non-high-income vs. high-income countries. Median age at familial hypercholesterolaemia diagnosis in adults with obesity was 9 years older than in normal weight adults. Obesity was associated with a more atherogenic lipid profile independent of lipid-lowering medication. Prevalence of coronary artery disease increased progressively across body mass index categories in both children and adults. Compared with normal weight, obesity was associated with higher odds of coronary artery disease in children (odds ratio 9.28, 95% confidence interval 1.77–48.77, adjusted for age, sex, lipids, and lipid-lowering medication) and coronary artery disease and stroke in adults (odds ratio 2.35, 95% confidence interval 2.10–2.63 and odds ratio 1.65, 95% confidence interval 1.27–2.14, respectively), but less consistently with peripheral artery disease. Adjusting for diabetes, hypertension and smoking modestly attenuated the associations. Conclusions Overweight and obesity are common in patients with HeFH and contribute to ASCVD risk from childhood, independent of LDL-C and lipid-lowering medication. Sustained body weight management is needed to reduce the risk of ASCVD in HeFH. © The Author(s) 2025

    Prevention of myopathies by statins, a literature review

    Las estatinas son fundamentales en el tratamiento de la dislipidemia y la prevención de enfermedades cardiovasculares, pero su uso puede verse limitado por la intolerancia o toxicidad. Entre estas complicaciones, las miopatías por estatinas varían en gravedad, desde mialgias leves hasta rabdomiólisis potencialmente fatal. Esta revisión bibliográfica realizó una exhaustiva búsqueda utilizando términos clave y bases de datos relevantes, siguiendo la metodología PRISMA. Se encontró que la prevalencia de miopatías por estatinas es baja, alrededor del 1%, pero la información disponible sobre este tema es limitada. La metodología de búsqueda incluyó la normalización de términos y la selección de artículos en español e inglés, con un límite temporal de 10 años. A pesar de la escasez de datos, se destaca la importancia de explorar alternativas para prevenir y tratar estas complicaciones, dada su significativa implicación en la seguridad y tolerabilidad del tratamiento con estatinas.PregradoQuímico FarmacéuticoStatins are essential in the treatment of dyslipidemia and the prevention of cardiovascular diseases, but their use may be limited by intolerance or toxicity. Among these complications, statin myopathies range in severity, from mild myalgias to potentially fatal rhabdomyolysis. This literature review carried out an exhaustive search using key terms and relevant databases, following the PRISMA methodology. The prevalence of statin myopathies was found to be low, around 1%, but the information available on this topic is limited. The search methodology included the standardization of terms and the selection of articles in Spanish and English, with a time limit of 10 years. Despite the scarcity of data, the importance of exploring alternatives to prevent and treat these complications is highlighted, given their significant implication in the safety and tolerability of statin treatment
