    A single-centre randomized-controlled trial to study effect of dilution on propofol-induced injection pain at injection site

    Background: Propofol is a commonly used short-acting intravenous anaesthetic agent. A major disadvantage of propofol is pain at injection site with high incidence up to 90%. Various modalities have been tried to obtund propofol-induced pain; however, search for an ideal agent continues. We assessed the effect of double and triple dilution of 1% propofol emulsion with normal saline on pain at injection site. Methods: This randomized, double-blinded study was done on 60 adult patients of both sexes, belonging to ASA grade I and II scheduled for elective surgery under general anesthesia, divided into three groups named I, II, III of 20 patients each. The patients of group I, II, and III received 1% propofol 2 ml, 0.5% propofol 4 ml, and 0.33% propofol 6 ml, respectively, over a period of 4 s and pain felt was assessed. Results: There was no statistically significant difference in the pain score in group II as compared to patients in group I. However, there was a statistically significant decrease in the pain score in group III as compared to patients in group I (P value 0.02) and group II (P value 0.03). Conclusions: We found a significant decrease in both incidence and severity of pain during injection with a 0.33% propofol solution without significant adverse hemodynamic effects. The small size of data was a limitation in our study and a large-scale study will be needed to prove its therapeutic beneficence

    A critical assessment of technical advances in pharmaceutical removal from wastewater – A critical review

    Use of pharmaceutical products has seen a tremendous increase in the recent decades. It has been observed that more than thirty million tons of pharmaceuticals are consumed worldwide. The used pharmaceutical products are not completely metabolized in human and animal body. Therefore, they are excreted to the environment and remain there as persistent organic chemicals. These compounds emerge as toxic contaminants in water and affect the human metabolism directly or indirectly. This literature review is an endeavour to understand the origin, applications and current advancement in the removal of pharmaceuticals from the environment. It discusses about the pharmaceuticals used in medical applications such diagnosis and disease treatment. In addition, it discusses about the recent approaches applied in pharmaceutical removal including microbial fuel cells, biofiltration, and bio nanotechnology approaches. Moreover, the challenges associated with pharmaceutical removal are presented considering biological and environmental factors.     Fuzzy logic based active vibration control using novel photostrictive composites

    Although conventional actuators like piezoelectric and electrostrictive are efficient, but they required hard wiring, which contaminates the control signal and adds to the weight of the structure. The current study presents a wireless control strategy using photostrictive actuators. Owing to the fortunate combination of photovoltaic effect and converse piezoelectric effect, a photostrictive actuator can generate mechanical strain, when irradiated with light intensity. Limited choices of photostrictive material with high electromechanical coupling coefficient give the motivation to design photostrictive composites. The finite element-based formulation incorporating fuzzy logic controller is employed to study the active vibration control response of cantilever structure when equipped with photostrictive composite actuator. A parametric study has been carried out to study the influence of inclusion's volume fraction on wireless active vibration control of the structure. Control merits have been defined to compare the control performance of different composites.     Humeral shaft fracture with ipsilateral shoulder dislocation

    【Abstract】Although fracture of the humeral shaft or dislocation of the shoulder joint is a common injury, a simutaneous injury is rare. We present such a case combined with head injury which took precedence over the skeletal injuries. The postoperative rehabilitation was slowed down by the head injury. This case report makes us aware of some problems when managing the patient with this rare injury and helps us understand the management options better. Also the need for proper follow-up and rehabilitation is emphasized
