    Role of serum Cystatin C as a marker of early nephropathy in metabolic syndrome: a case control study

    Background: The metabolic syndrome (MS) has become a significant public health problem and patients with MS are at higher risk for developing renal diseases. Serum Cystatin C suggested as a sensitive endogenous marker than creatinine for slight changes in GFR could be useful marker in MS.Methods: A total of 200 subjects were included. New International Diabetes Federation (IDF) definition of MS was used as inclusion criteria. Patients excluded were those with hypo/hyperthyroidism, on glucocorticoids, statins and fibrate, malignancy, cirrhosis, active liver disease and conditions affecting abdominal girth. Serum Cystatin C, insulin, creatinine, triglycerides, high density lipoproteins-cholesterol (HDL-C), fasting glucose, Urinary microalbumin and Urinary creatinine were estimated by standard method.eGFR and HOMA-IR (homeostasis model assessment of insulin resistance) were calculated. The primary outcome assessed was the occurrence of early nephropathy in MS and the secondary outcome included evaluation of early nephropathy by serum Cystatin C and eGFR. Appropriate statistical test was applied by using SPSS Version 21 software.Results: Fasting insulin levels and insulin resistance were significantly raised in MS cases. eGFR (MDRD) was lower in the MS cases (72.59±8.79mL/min/1.73m2) vs non-MS (130.34±40.75 mL/min/1.73m). Urinary microalbumin levels and serum cystatin C were significantly increased in MS and the cystatin c levels showed significant positive correlation with urinary microalbumin and negative correlation with eGFR.eGFR was found to be lower in the microalbuminuric than normoalbuminuric groups.Conclusions: Serum Cystatin C levels are higher in MS and can be useful, practical, non-invasive biomarker for evaluation of early renal involvement in MS.

    Quality control in healthcare system is still less understood because of the relative complexity in choosing an appropriate Westgard rule .Six Sigma methodology is a manufacturing strategy first pioneered by Motorola Company in 1980s, with the goal of decreasing the defect rates in production. It has improved the production efficiency of different industries. To achieve the similar high quality and near zero defect rates in healthcare system, six sigma metrics is being used in many clinical laboratories and diagnostic industry. Six sigma metrics is used in combination with total allowable error (CLIA ’88 proficiency testing criteria), method imprecision and bias. The goal is to attain the highest possible sigma scale within the acceptable limits of total allowable error. This article reviews the different Westgard control rules and their implications in pointing towards a particular error, as well as basic principles of Six Sigma methodology & their practical utility in the clinical laboratory

    Role of clinical and radiological parameters in predicting the outcome of shockwave lithotripsy for ureteric stones

    Introduction: Shockwave-lithotripsy (SWL) provides a noninvasive and effective option for the management of ureteric calculi. Several factors may affect the success of SWL. Identification of these predictive factors will both increase the efficacy and decrease the cost. This study was designed to identify factors affecting the outcome of SWL for ureteral stones. Materials and Methods: This study was conducted from March 2012 to November 2014 in patients with solitary ureteric calculi who were managed with SWL. Data were analyzed to identify clinical and radiological factors associated with treatment outcome. Success after SWL was described as complete stone clearance or clinically insignificant residual fragments <3 mm at 3 months after SWL. Results: A total of 110 patients with ureteric calculi were divided into two groups depending on the outcome of SWL, Group A (successful - 76%) and Group B (failed - 24%). Stone size, Skin to stone distance (SSD), secondary signs of obstruction, and presence of double J (DJ) Stent, all were significantly associated with the outcome of SWL on univariate analysis. On multivariate analysis, stone size, hounsfield unit, SSD, and DJ stent were the independent factors affecting the outcome of SWL. On Receptor-Operator Characteristic curve analysis, a cutoff value of 8.2 mm for the stone size was found which best predicts a successful outcome, with a sensitivity of 54% and specificity of 96%. Conclusion: The findings of this study suggest that Stone size, SSD, the presence of DJ stent, and stone attenuation values are the significant factors that influence the outcome of SWL in patients with ureteral stones

    Assessment of Community Behavior and COVID-19 Transmission during Festivities in India: A Qualitative Synthesis through a Media Scanning Technique

    In India during the first wave of COVID-19 infection, the authorities were concerned about the advent of the festive season, which could lead to a surge in cases of SARS-CoV-2 infection. The present study attempted to assess the socio-behavioral aspects of COVID-appropriate behavior (CAB) at individual and community levels, and their impact on the transmission of COVID-19 during festivities in India. Media scanning was conducted to qualitatively assess CAB by analyzing 284 news reports from across India; quantitative data on COVID-19 daily cases from March 2020 to December 2020 were used to determine the trends of the adjusted test positivity (ATP) ratio for six cities. Peaks in ATP were observed in Chandigarh, Delhi-NCR in North India during and after Dussehra and Deepavali, and in Mumbai, in the west, after Navratri. Additionally, a surge in ATP was observed in Trivandrum after Onam and in Chennai around Deepavali in the south; meanwhile, in the eastern city of Kolkata, cases increased following Durga Puja. The major challenges were adherence to CAB viz. social distancing, hygiene, and compliance with the mask mandate. Microlevel CAB indicated relatively higher laxity in maintaining hand hygiene in all cities. Observations from the current study indicate that innovative community-driven initiatives during festivals in each geographical zone are key to the large-scale implementation of disease prevention measures

    Social safety net and the poor during the transition : the case of Bulgaria

    Using data from the 1992 Bulgarian household budget survey, the authors analyze the structure of income in Bulgaria, identifying who the poor are and how they are reached by the social safety net. Their main findings about household incomes: (a) Social transfers provide an extremely large component - 24 percent - of household income per capita. That is roughly on a par with the share in other Eastern European countries but more than 40 percent higher than the share of the Organization for Economic Cooperation and Development (OECD) countries. (b) Wage earnings have declined as a source of income, reflecting the counteraction of the state sector. Wage income in Bulgaria has declined to only half the OECD level. (c) Income from self-employment has increased, reflecting the surge in small-scale retail establishments. Income is considerably less concentrated in Bulgaria than in other lower-middle-income countries. The author's main findings about the poor (the bottom 20 percent in terms of household income): The head of household in a poor home tends to be older, a woman, poorly educated, and unemployed. Poor households are not necessarily larger households in Bulgaria, unlike in other developing countries. The sources of income in poor Bulgarian households reflect other findings: (a) The poor depend for more than half their income on social benefits (especially pensions), indicating the importance of the social safety net. (b) The social safety net is not well targeted. Most social benefits are pro-poor, in the sense that they improve income distribution, but many benefits accrue to better-off households. There is substantial scope for better distribution of income.     This study was conducted to investigate the validity and reliability of the Fremantle Neck Awareness Questionnaire (FreBFA) in Turkish with chronic neck pain individuals. The main questionnaire is "The Fremantle Back Awareness Questionnaire" and Prof. Benedict Wand who the actual questionaire writer permission was obtained. The questionnaire was converted by the author into a neck version and the neck version of the questionnaire was translated into Turkish. The patients were assessed with demographic information, severity of pain, Neck Disability Index (NDI), Tampa Kinesiophobia Scale (TKS), SF36 Quality of Life Questionnaire (SF36), Beck Depression Scale (BDS) and Beck Anxiety Scale (BAS). Test-retest evaluations of 37 patients were performed on our unit 3 days after the first application. Internal consistency Cronbach Alpha, ICC (Intraclass Correlation Coefficient), factor analysis in validity assessment, and the severity of pain with FreBFA, NDI, TKS, SF36, BDS and BAS questionnaires were examined in the reliability evaluation. FreBFA was evaluated in 111 patients with chronic neck pain. The internal consistency of FreBFA was found to be acceptable. When analyzed by Scree Plot graph, the questionnaire was found to have four factors. Questions 1, 2 and 3 are attention; 4 and 5 are proprioceptive sense; 6, 7, and 9 questioned the dimensions of the senses of neck shape and size, question 8 is ignore. When FreBFA and other questionnaire relations were examined, there were relations between low and moderate level (p <0.05). FreBFA is a valid and reliable questionnaire for individuals with chronic neck pain. The questionnaire was first carried out in the area of awareness and in the neck region and for the first time in Turkish. Hastaların demografik bilgileri, ağrı şiddetleri, Boyun Özür Anketi (BÖA), Tampa Kinezyofobi Ölçeği (TKÖ), SF36 Yaşam Kalitesi Anketi (SF36), Beck Depresyon Ölçeği (BDÖ) ve Beck Anksiyete Ölçeği (BAÖ) ile değerlendirmeleri yapıldı. FreBFA 111 kronik boyun ağrılı hastada değerlendirildi. 37 hastaya test-tekrar test değerlendirmesi ilk uygulamadan 3 gün sonra ünitemizde yapıldı.     Comparison of the ottawa ankle rules to ankle ultrasound imaging in the decision making process for the indication of radiographic imaging of the ankle

    Amaç: Ayak bileği travmaları acil servis başvurularının büyük çoğunluğunu oluşturmaktadır. Acil servise başvuran ayak bileği travmalarının %15 inde fraktür saptanmaktadır; yani bütün ayak bileği travmalarının X-ray görüntüleme gerekliliği yoktur. X-Ray endikasyonu belirlemede yaygın olarak kullanılan OAK’ın yüksek duyarlılık ve düşük özgüllüğe sahip olması nedeniyle başka yöntemler gündeme gelmektedir. USG noninvaziv, taşınabilir, ucuz ve zararsız olması nedeni ile birçok çalışmada kemik yapılar için tanısal bir araç olarak görülmüştür. Çalışmamızda, temelde OAK ve USG’nin fraktür öngördürmede birbirlerine üstünlükleri, yanı sıra USG’nin ayak bileği kırıkları için tanısal değerinin araştırılması amaçlanmıştır. Materyal-Metod: Çalışmada Ege Üniversitesi Tıp Fakültesi Acil Servisine Eylül 2017 -Mart 2018 tarihleri arasında ayak bileği travması ile başvuran hastalar arasından 18 yaş üstü, cinsiyet ayrımı olmaksızın seçilen 285 hasta incelendi. Hastaların acil tıp asistan hekimi tarafından yapılan ilk muayenesinde OAK sorgulandı. Gönüllü Onam formunu imzalayan hastaların hepsine ayak bileği USG incelemesi yapıldı. Hastaların USG değerlendirmeleri, 2 yıl üstü kıdemde, çalışma öncesinde otör uygulayıcı ile iyi ve çok iyi uyum sağlayan 5 kişilik acil tıp asistan ekibi tarafından yapıldı. İncelemeler sırasında Sonosite-Fujifilm portable ultrason 6-13M hz lineer prob kullanıldı. Yine tüm hastalara OAK ve USG sonuçlarından bağımsız X-Ray görüntülemesi yapıldı. Sonuçlarından habersiz acil servis ortopedi konsültan hekimi, hastaları değerlendirip kesin tanıyı belirledi. Veriler olgu rapor formuna kaydedilerek uygun istatistiksel yöntemlerle karşılaştırıldı. Bulgular: Çalışmamızda ayak bileği travması ile başvuran ve çalışma kriterlerini karşılayan 285 hastaya OAK, USG ve X-Ray ile yapılan değerlendirmeler sonucunda elde edilen verilere göre OAK’ın negatif prediktif değeri yüksek olmasına rağmen pozitif prediktif değeri oldukça düşük bulundu (sırasıyla %100; %27,9). Çalışmaya katılan hastalara, 5 farklı uygulayıcı tarafından yapılan ayak bileği USG sonuçlarına göre fraktür tespitinde PPD %94,0, NPD %94,5 bulundu. USG, fraktür tespitinde özgüllüğü açısından OAK’a üstün bulundu (p= 0,036). Tüm hastalara uygulanan X-Ray’in ayak bileği kırıklarının tespitinde duyarlılığı %96,0, özgüllüğü %97,6 bulundu. X-Ray, USG ile kıyas edildiğinde, USG ‘nin özgüllüğünün daha yüksek ve duyarlılığının daha düşük olduğu bulundu (sırasıyla %84,0; %98,1, %96,0; %97,6). USG ile X-Ray ‘in fraktür tespitindeki kappa değeri ise 0,850 bulundu. Çalışmaya katılan hastalarda yaş ve cinsiyet açısından USG’nin tanı koyduruculuğu kıyaslandığında istatistiksel olarak farklılık bulunmadı. Çalışmamızda X-Ray’in tanı koyamadığı ligament hasarları da değerlendirilmiş olup, literatürle uyumlu olarak %12,4 oranında ATFL hasarı tespit edildi. USG’nin ligament hasarı tanısı koymada özgüllüğü %97,6 bulundu. Sonuç: OAK, ayak bileği travmalarında görüntüleme öncesinde uygulanabilecek, duyarlılığı yüksek bir incelemedir. Gereksiz grafi çekimlerinin önüne geçmek için USG ile OAK’ın özgüllüğü artırılabilir. Tek başına USG, bu noktada OAK’a üstündür. Noninvaziv, taşınabilir ve radyasyon barındırmayan bir işlem olması nedeni ile USG, tekrarlayan görüntülemeye imkan sağlar ve atlanan fraktürlerin tespitinde kullanılabilir. USG ile fraktür tanısı için kadın-erkek, yaş açısından fark yoktur. Ayak bileği USG bakısı, grafi çekilemeyen hastaların fraktür tanı ve tedavisinde güvenle kullanılabilir. USG ile ayak bileğinin yumuşak doku ve ligament yaralanmalarını tespit etmek mümkündür; fakat özellikle ligament hasarı tespitinde güvenliği için yeni çalışmalara ihtiyaç vardır.Objective: Ankle traumas account for a major proportion of emergency department admissions. In the United States, 28,000 emergency department presentations with ankle traumas have been recorded per day. 15% of the ankle trauma cases presenting to the emergency department services are diagnosed with fractures, which suggests that not all ankle traumas require radiographic imaging. To determine which cases will require ankle X-Rays, Ottowa ankle rules (OAR) provide instructions in line with the current literature. However, due to the high sensitivity and lower specificity of OAR, the requirement for other methods is a current issue. Ultrasound (US) is non-invasive, portable, less costly, and hazardless, therefore, it is suggested as a diagnostic tool for the evaluation of bone structures in many studies. In our study, it was aimed to compare the OAR and US imaging in predicting the presence of fractures and to determine the diagnostic value of the US in ankle fractures. Material and Method: A total of 285 female and male patients older than 18 years old, who presented with ankle traumas to the emergency department of Ege University Medical Faculty between September 2017 and March 2018, were studied. At the initial physical examination, which was performed by the emergency department resident, OAR was questioned. US imaging of the ankle was performed for all patients who signed the consent forms to participate the study. The assessments of the US images were performed by an investigator team consisting of 5 senior residents of emergency department with more than 2 years of residency period and who provided good and outstanding agreements with the US supervisor/trainer, the emergency care specialist, and the author investigator prior to the study. The investigations were performed with Sonosite-Fujifilm portable ultrasound 6-13M Hz linear probe. X-ray images were obtained from all patients in the study regardless of the OAR and US findings. An emergency care consultant orthopedist, who was uninformed of any of the findings, evaluated the patients and made the final diagnosis. All findings were recorded in the case report forms and the data were compared using appropriate statistical methods. Results: A comprehensive analysis has been performed evaluating the findings of OAR, US, and X-ray from 285 patients presenting with ankle traumas and complying with the study criteria. In this study, which we aimed to investigate the reliability of OAR in decision making to obtain X-rays, the positive predictive value (PPV) was remarkably low despite the high negative predictive value (NPV) (27.9% and 100% respectively). In order to determine the fractures, the ankle US of the study patients were performed by 5 different investigators, yielding a PPV of 94.0% and an NPV of 94.5%. In terms of specificity, US imaging was superior to OAR in determining the fractures (p= 0.036). Ankle X-Rays were obtained from all patients and yielded a 96.0% sensitivity and 97.6% specificity in determining the ankle fractures, representing similar limitations with the literature. When these findings were compared with the US, the specificity of US was found to be higher, whereas its sensitivity was lower (84.0%, 98.1%, 96.0%, 97.6% respectively). The kappa value was 0.850 for US and X-Ray in determining the fractures and this value was evaluated to indicate a good concordance. No statistical differences were found when the age and gender of the study patients were compared in terms of making a diagnosis with the US imaging. In our study, the ligament injuries, which are not diagnosed with X-Ray images, were evaluated, too, and ATFL injuries were detected at a rate of 12.4%, consistent with the reports in the literature. The specificity of US was 97.6% and consistent with the literature in making the diagnosis of ligament injuries. Conclusion: OAR can be applied before radiological imaging in ankle traumas and allows investigation findings with high sensitivity. The screening of the ankle trauma patients with OAR and US can be a method to be employed to prevent unrequired X-ray imaging. US alone is superior to OAR in this respect. Being a noninvasive, portable and radiation-free procedure, US allows repeat investigations and therefore can be used in diagnosing the missed fracture cases. Diagnoses made with US images do not demonstrate any differences in terms of gender or age. The US examinations of the ankle can be used reliably in making the diagnosis and the treatment of fractures. Making the diagnosis of soft tissue and ligament injuries is possible with US in patients with ankle traumas, however, further studies are required in determining their reliability particularly in detecting the ligament injuries
