Microvascular integrity after reperfusion therapy
Abstract
Several perfusion techniques have definitively shown that microvascular dysfunction plays a crucial role in patients with acute myocardial infarction. In those patients, despite a rapid and sustained restoration of flow throughout a previously occluded epicardial coronary artery, microvascular damage still may be observed. Duration of ischemia and/or time to recanalization are the most powerful determinants of microvascular dysfunction. However, the amount of tissue perfusion in infarcted patients is dependent on many other complex interrelated factors including extent of collateral circulation before recanalization, residual stenosis severity of the culprit artery, vasodilator reserve in the infarct territory, extent of reperfusion injury, and loading conditions. Because microvascular dysfunction is associated with progressive left ventricular dilation and a high frequency of postinfarction complications, all of the efforts to improve the relation between coronary reflow and microvascular perfusion are justified

Viene illustrata l’esperienza di un cardiologo e diuna internista in Yemen, con istantanee dalla vita quotidiana, dentro e fuori gli ospedali. Dopo avere ripercorso le tappe principali della cooperazione Italia-Yemen, viene descrittala situazione della sanità yemenita, con particolare riferimento ai suoi ospedali e alla preparazione del personale medico e alle differenze di genere nell’approccio alle pazienti e ai pazienti. Vengono poi presi in considerazione alcuni aspetti caratterizzanti la stato di salute della popolazione dello Yemen, come l’altissima incidenza di cardiopatia reumatica a causa della mancanza totale di prevenzione primaria esecondaria e le differenze di genere nel trattamento delle donne con sindrome coronarica acuta.This paper illustrates the experience of a cardiologist and an internist in Yemen, with snapshots from everyday life, both inside and outside hospitals. After outlining the main stages of Italy-Yemen cooperation, it describes the situation of Yemeni health care, with particular reference to its hospitals, the training of medical personnel and gender differences in the approach to male and female patients. It then takes into consideration certain aspects characterising the state of health of the Yemeni population, such as the extremely high incidence of rheumatic heart disease due to the total lack of primary and secondary prevention and gender differences in the treatment of women with acute coronary syndrome
