    A Businger Mechanism for Intermittent Bursting in the Stable Boundary Layer

    High-resolution large-eddy simulations of the Antarctic very stable boundary layer reveal a mechanism for systematic and periodic intermittent bursting. A nonbursting state with a boundary layer height of just 3 m is alternated by a bursting state with a height of ≈5 m. The bursts result from unstable wave growth triggered by a shear-generated Kelvin–Helmholtz instability, as confirmed by linear stability analysis. The shear at the top of the boundary layer is built up by two processes. The upper, quasi-laminar layer accelerates due to the combined effect of the pressure force and rotation by the Coriolis force, while the lower layer decelerates by turbulent friction. During the burst, this shear is eroded and the initial cause of the instability is removed. Subsequently, the interfacial shear builds up again, causing the entire sequence to repeat itself with a time scale of ≈10 min. Despite the clear intermittent bursting, the overall change of the mean wind profile is remarkably small during the cycle. This enables such a fast erosion and recovery of the shear. This mechanism for cyclic bursting is remarkably similar to the mechanism hypothesized by Businger in 1973, with one key difference. Whereas Businger proposes that the flow acceleration in the upper layer results from downward turbulent transfer of high-momentum flow, the current results indicate no turbulent activity in the upper layer, hence requiring another source of momentum. Finally, it would be interesting to construct a climatology of shear-generated intermittency in relation to large-scale conditions to assess the generality of this Businger mechanism.Atmospheric Remote Sensin

    On-table reconstruction of comminuted fractures of the radial head

    The most widely accepted treatment for comminuted fractures of the radial head is either the excision or open reduction and internal fixation. The purpose of the present study is to evaluate the value of an 'on-table' reconstruction technique in severely comminuted fractures of the radial head. In this study, two patients with a Mason type-III and four patients with a Mason type-IV radial-head fracture were treated with 'on-table' reconstruction and fixation using low-profile mini-plates. After a mean follow-up of 112 months (47-154 months), the mean elbow motion was 0-6-141 degrees extension flexion with 79 degrees of pronation and 70 degrees of supination. The mean Broberg and Morrey functional rating score was 97.0 points, the Mayo Elbow Performance Index was 99.2 points and the mean Disabilities of the Arm, Shoulder, and Hand (DASH) Outcome Measure score was 1.94 points. One patient had symptoms of degenerative changes, with a slight joint-space narrowing. There were no radiographic signs of devitalisation at final examination. Comminuted fractures of the radial head, which would otherwise require excision, can be successfully treated with an 'on-table' reconstruction technique

    Prevalence of burnout among surgical residents and surgeons in Switzerland

    Burnout is a pathologic reaction in response to long-term work-related stress. The aim of this study was 2-fold: first, to assess the prevalence and degree of burnout among surgical residents and surgeons in Switzerland and, second, to identify predictors of burnout in the surgical community. Four hundred five of 618 anonymous questionnaires (65.5%) were returned. Among respondents, 3.7% and 35.1% showed high and moderate degrees of burnout, respectively. Respondents with high and moderate degrees of burnout had higher summary scores of perceived stress (P < .001). In multiple logistic regression analysis, the strongest predictors of burnout were poor interaction with nurses, disturbances due to telephone consultations, and high overall workload. To reduce burnout, new work models should be sought, in addition to decreasing work intensity and workload rather than restricting work hours alone

    Women in surgery: a survey in Switzerland

    An increasing proportion of women work in medicine; however, only few choose surgical specialties. The objective of this study was to analyze the current situation of female surgeons and surgical residents in Switzerland concerning their personal and professional fulfillment. Of 318 female surgeons and surgical residents included in our study, 189 (59.4%) returned the anonymous questionnaire. Mentor-mentee relationships were mentioned by 110 (58.2%) of the 189 respondents. On the basis of a 7-point Likert scale, these women responded that they were moderately satisfied with their professional (mean score [SD], 2.7 [1.3]) and personal (mean score [SD], 3.0 [1.7]) lives. Of the 189 respondents, 113 (59.8%) mentioned that they felt underappreciated. The most important ways suggested for increasing the attractiveness of a surgical career for women were a reduction in workload (49 respondents [25.9%]), more flexible working hours (38 respondents [20.1%]), and better structured residency programs (23 respondents [12.2%])

    Demographic situation in operative medicine in Switzerland - time to react?

    Objective: The decreasing proportion of physicians of Swiss origin and the increasing number of part-time jobs in operative medicine might lead to a shortage of physicians in operative disciplines in Switzerland. The objective of the present study was to analyze the current demographic situation in operative medicine in Switzerland. Methods: During the summer of 2011, a 19-item anonymous electronic questionnaire was mailed to all directors of departments in operative medicine in Switzerland. The questionnaire was designed to gather data about the characteristics of the participating departments, the demographics (including the appointment (consultant, attending or resident), the proportion of female and foreign physicians, the latter’s origin, and the number of part-time jobs with a working time between 20 and 90%), and the proportion of vacant posts. Results: Of 775 questionnaires mailed to all directors of departments in operative medicine in Switzerland, 183 (24%) were returned. Overall, 40% were female, and 42% foreign physicians. The proportion of part-time jobs amounted to 17%. Vacant posts were found in 2%. Conclusions: An expansion of study places at the medical universities and of the incentives for the incumbents in operative medicine is necessary to avert a shortage of physicians in Switzerland
