No full textBackground: Type 2 diabetes mellitus (T2DM) induces a hypercoagulable state that increases thrombotic risk.
Objective: This study evaluated coagulation parameters and platelet indices in Yemeni adults with T2DM and their correlation with glycemic control.
Methods: A hospital-based cross-sectional study was conducted on 140 T2DM patients and 100 healthy controls from major hospitals in Aden Governorate between January and February 2025. Coagulation tests (PT, APTT) were performed using STA-R Evolution, and platelet indices (MPV, PDW) were analyzed via Sysmex XN-550. Glycemic control was assessed by HbA1c. Statistical analysis was performed using SPSS v26.
Results: Most patients (86%) had poor glycemic control (HbA1c ≥ 7%). Diabetic patients demonstrated significantly prolonged prothrombin time (PT) compared with healthy controls (13.69 ± 1.74 sec vs. 12.10 ± 1.20 sec, p < 0.001), shortened APTT (31.38 ± 4.16 sec vs. 35.20 ± 3.50 sec, p < 0.001), and elevated MPV (9.03 ± 0.92 fL vs. 8.70 ± 1.10 fL, p = 0.015) and PDW (16.8 ± 2.1% vs. 15.2 ± 1.8%, p = 0.01) compared to controls. A strong positive correlation was found between HbA1c and MPV (r = 0.52, p < 0.001). An MPV cut-off > 11.5 fL predicted thrombotic risk with 78% sensitivity and 85% specificity (AUC = 0.82).
Conclusion: Yemeni T2DM patients demonstrate significant hemostatic abnormalities strongly linked to poor glycemic control. MPV represents a cost-effective, accessible marker for thrombotic risk stratification. We propose its integration into routine diabetic care protocols in Yemen and similar resource-limited settings.

NON-ALCOHOLIC FATTY LIVER DISEASE (NAFLD): A REVIEW OF PATHOPHYSIOLOGY AND CLINICAL MANAGEMENT
No full text<p><i><strong>Introduction: </strong>The harmful effects of non-alcoholic fatty liver disease (NAFLD) are becoming a significant problem for public health due to the rising incidence of diabetes and obesity worldwide. In the Western world, NAFLD is the most prevalent chronic liver disease. Metabolic problems such as central obesity, dyslipidemia, hypertension, hyperglycemia, and recurrent abnormalities in liver function tests are all intimately related to NAFLD. In general, NAFLD is a term used to describe a wide range of liver diseases, including fibrosis, inflammation, and hepatocyte injury. This is typically detected through a liver biopsy and can take a variety of forms, from lesser forms (steatosis) to more severe forms (cirrhosis, advanced fibrosis, non-alcoholic steatohepatitis, and liver failure). A precise diagnosis of NASH and NAFLD is essential since severe fibrosis is the main indicator of morbidity and liver-related death in these patients. The gold standard for diagnosing NAFLD continues to be histologic assessment with liver biopsy. The presence of hepatic steatosis, ballooning, and lobular inflammation with or without fibrosis is required for the diagnosis of NAFLD. Once the diagnosis is made, the pillars of treatment are still weight loss, dietary changes, and the management of the underlying metabolic syndrome. Once a diagnosis is made, the fundamentals of therapy continue to be dietary advice and lifestyle changes, weight loss, and the treatment of the underlying metabolic syndrome, all of which show promise but are challenging to sustain. Guidelines prescribe pioglitazone and vitamin E in some people.</i></p><p><i><strong>Aim of the study: </strong>Clinicians encounter difficulties due to the complexity of NAFLD and related diseases, especially its association with metabolic syndrome. The pathophysiology of NAFLD, risk factors, diagnostic techniques, and conservative and surgical treatment options are all summarised in this review. An overview of NAFLD and available treatments is provided in this review.</i></p><p><i><strong>Methodology: </strong>The present review is a comprehensive research of PUBMED since the year 1995 to 2023</i></p><p><i><strong>Conclusion: </strong>The management of these patients has grown more challenging due to the expanding obesity pandemic and the increased frequency of concomitant disorders, including T2DM and NAFLD. There are several therapy options; however, there isn't much high-quality research that compares them with one another. Prospective studies addressing the remaining uncertainties regarding the relationship between insulin resistance, fatty liver, and fibrosis progression should be made accessible soon, given the rising popularity of bariatric surgery.</i></p><p><i><strong>Keywords: </strong>Non-alcoholic fatty liver disease (NAFLD), obesity, dyslipidemia, hypertension, hyperglycemia, liver failure etc.</i></p>
Mapping artificial intelligence adoption in hepatology practice and research: challenges and opportunities in MENA region
No full textBackgroundArtificial intelligence (AI) is increasingly relevant to hepatology, yet real-world adoption in the Middle East and North Africa (MENA) is uncertain. We assessed awareness, use, perceived value, barriers, and policy priorities among hepatology clinicians in the region.MethodsA cross-sectional online survey targeted hepatologists and gastroenterologists across 17 MENA countries. The survey assessed clinical and research applications of AI, perceived benefits, clinical and research use, barriers, ethical considerations, and institutional readiness. Descriptive statistics and thematic analysis were performed.ResultsOf 285 invited professionals, 236 completed the survey (response rate: 82.8%). While 73.2% recognized the transformative potential of AI, only 14.4% used AI tools daily, primarily for imaging analysis and disease prediction. AI tools were used in research by 39.8% of respondents, mainly for data analysis, manuscript writing assistance, and predictive modeling. Major barriers included inadequate training (60.6%), limited AI tool access (53%), and insufficient infrastructure (53%). Ethical concerns focused on data privacy, diagnostic accuracy, and over-reliance on automation. Despite these challenges, 70.3% expressed strong interest in AI training., and 43.6% anticipating routine clinical integration within 1–3 years.ConclusionMENA hepatologists are optimistic about AI but report limited routine use and substantial readiness gaps. Priorities include scalable training, interoperable infrastructure and standards, clear governance with human-in-the-loop safeguards, and region-specific validation to enable safe, equitable implementation
A remote-controlled global navigation satellite system based rover for accurate video-assisted cadastral surveys
No full textOne of the main tasks of a cadastral surveyor is to accurately determine property boundaries by measuring control points and calculating their coordinates. This paper proposes the development of a remotely-controlled tracking system to perform cadastral measurements. A Bluetooth-controlled rover was developed, including a Raspberry Pi Zero W module that acquires position data from a VBOX 3iSR global navigation satellite system (GNSS) receiver, equipped with a specific modem to download real-time kinematic (RTK) corrections from the internet. Besides, the Raspberry board measures the rover speed with a hall sensor mounted on a track, adjusting the acquisition rate to collect data at a fixed distance. Position and inertial data are shared with a cloud platform, enabling their remote monitoring and storing. Besides, the power supply section was designed to power the different components included in the acquisition section, ensuring 2 hours of energy autonomy. Finally, a mobile application was developed to drive the rover and real-time monitor the travelled path. The tests indicated a good agreement between rover measurements and those obtained by a Trimble R10 GNSS receiver (+0.25% mean error) and proved the superiority of the presented system over a traditional metric wheel
