Acute Acalculous Cholecystitis in an Outpatient Setting
Acute acalculous cholecystitis (AAC) typically affects hospitalized
patients with critical illness. Outpatient AAC is reported to occur in
elderly males with cardiovascular comorbidities. We report the
presentation of acute acalculous cholecystitis in two young African men
admitted for the first time within days of each other. They reported
short histories of right upper abdominal pain and had tender
gallbladders. Sonography revealed gallbladder wall thickening. Open
cholecystectomy and histology confirmed AAC.The postoperative outcome
was excellent. Recognition of AAC in a subset of young male patients
presenting de novo with right upper quadrant pain is stressed.
Cholecystectomy has good prognosis in these cases

LOWER LIMB AMPUTATIONS AT THE KENYATTA NATIONAL HOSPITAL, NAIROBI
Background: Lower limb amputations continue to be widely performed despite attempts to managerisk factors such as diabetes mellitus and peripheral vascular diseases. The incidence of vascularmajor lower limb amputations is higher in the developed countries than that reported in thedeveloping ones mainly due to the ageing population. In developing countries, poor access tobasic healthcare due to poverty impacts negatively on health seeking behaviour with conditionsotherwise preventable or easily manageable complicating to warrant amputation. There has been arise in the number of patients with diabetes being managed at Kenyatta National Hospital (KNH).However, the impact of this disease on the overall rates and pattern of lower limb amputations inKNH has not been studied.Objective: To determine the causes and pattern of lower limb amputations at the Kenyatta NationalHospital.Design: Descriptive prospective study.Setting: The Kenyatta National Hospital between July 2003 and June 2004.Results: A total of 77 lower limb amputations (LLA) were performed on 74 patients. The age rangedfrom seven months to 96 years (mean 44.8 ± 22.5). Forty six patients (62.1%) were males. Majority ofthe patients (89.1%) had primary or no formal education, forty one (55.4%) were unemployed, with39% self employed in the informal sector. Peripheral vascular diseases were the main indication forLLA (55.3%), 13 patients (17.5%) due to diabetes-related gangrene. Eighteen patients (24.3%) hadtumours, mainly osteogenic sarcoma (16.2%), while trauma accounted for 18.9%. Forty two (55%)of the amputations were above-the-knee, 24 (31%) below-the-knee, four (5%) hip disarticulationsand seven (9%) were foot amputations.Conclusion: This study found peripheral vascular diseases unrelated to diabetes to be the mainindication for lower limb amputations at Kenyatta National Hospital contrary to previousinstitutional and loco-regional studies which report trauma as the leading cause. Furtherinvestigation into vascular causes is therefore recommended
Selecting Robots to Take Over Tasks in Hospitality Settings: Joining Two Research Fields
Robotic assistance for work processes in the hospitality industry is receiving increased attention both in the hospitality industry and in academic research. Unfortunately, academic literature about hospitality robotics is currently disjointed, making it hard for hospitality professionals to decide which processes to target, and which available robotic systems would result in benefits (or limitations) for the organisation, employees or guests. Similarly, it is hard to understand what robotic functionalities need to be developed for particular processes, or what future functionalities to expect from ongoing developments in the robotics field. Researching hospitality robotics requires expertise in both fields. With this chapter, we try to give an insight in the things that should be considered when designing and evaluating the effectiveness of robots in a hospitality work environment. Investigation of the relation between growth mechanism and material properties of amorphous silicon solar cells
Thin film silicon solar cells are produced by using plasma deposition techniques. With this technique a thin film layer of silicon, most commonly amorphous silicon (a-Si: H), can be deposited on a substrate. The material properties depend largely on the plasma properties and the growth mechanism. The investigation of the correlation between material properties and the ion bombardment, which are important for electronic device operation, is the sole aim of this thesis. Expanding Thermal Plasma Chemical Vapour Deposition (ETP-CVD) is used for the deposition of a-Si: H layers. With this deposition technique, growth rates in excess of 1nm/s can be achieved. Through this, the production cost of solar cells can be reduced because of the increase in the production output. The growth mechanism of a-Si:H layers deposited with this technique has been extensively investigated and it has been shown that SiH3 radicals contribute to about 90% of the growth. The growth mechanism of a-Si: H layers can be manipulated by applying a bias to the substrate. In this way, the contribution of ions to the growth and the ion energy can be varied. To induce ion bombardment during film growth, a non-sinusoidal pulse-shaped signal is applied to the substrate and the material properties, which include optical properties, structural properties, and electrical properties, are measured. Denser a-Si:H films are obtained with increasing ion energy, indicating material densification. Higher growth rate is observed because of the production of more radicals around the substrate. An increase in photoconductivity is obtained up to 100V, which correlates with the decrease in the Tauc band gap and the densification of the material. Increase in the ion bombardment leads to decrease in the microstructure, which indicates reduction of voids in the material. However, defect density and Urbach energy increases with the ion bombardment due to the creation of weak bonds which end up as defects in the a-Si:H films. The optimized layers of the measured samples are incorporated into solar cells. Solar cells that are produced with pulse-shaped bias voltage up to 50V yielded a conversion efficiency of 6.2%. The efficiency is lower at higher voltages, which is mainly caused by smaller Voc and FF because of high defect densities present in the intrinsic layers at higher voltages.MicroelectronicsElectrical Engineering, Mathematics and Computer Scienc
