Abstract
BACKGROUND:
Fibrous hamartoma (FH) of infancy is a benign mesenchymal tumor, occurring as a superficial mass. Complete excision is curative.
OBJECTIVE AND METHODS:
The clinical features and treatment results of 18 children with FH are described.
RESULTS:
Local excision was the most common procedure. Surgery was radical in 10 patients, with microscopic residual disease in 6; all of them are alive with no evidence of disease 2 to 49 months after diagnosis. One patient, treated with a local reexcision for macroscopic residual disease (and chemotherapy for a synchronous desmoid fibromatosis) is well 83 months after diagnosis; the last patient, with a lesion of the labia majora, only underwent biopsy and is doing well, awaiting plastic surgery.
LIMITATIONS:
The results did not reach statistical significance due to difficulties in collecting cases.
CONCLUSIONS:
FH should be treated by complete excision; in our experience a nonradical excision was also able to achieve the cure. An aggressive approach should be avoided, as the overall prognosis is excellent.

Probabilistic Comparative Analysis of Vehicle-Bridge Interaction Models for Predicting Bridge Response under Moving Vehicles
This paper compares three approaches for predicting the bridge response under moving vehicles. The first approach represents the vehicle as a concentrated moving load, while the second and third approaches simulate the vehicle using single (1-dof) and two (2-dof) degrees of freedom models, respectively. The vehicle-bridge models have been implemented following a finite-difference formulation. These models have been validated against the experimental displacement response of a mid-span prestressed reinforced concrete bridge. Parametric analyses have been conducted to investigate the impact of road roughness on the discrepancies between the models' predictions. An increasing level of road roughness, modeled according to ISO-8608, tends to conceal the mutual differences between the models' outcomes. Furthermore, a Monte Carlo simulation has been employed to fit the probability distributions of two indicators, comparing the bridge displacement responses in three roughness scenarios and considering variable velocity and vehicle characteristics. The primary objective is to assess whether there are scenarios where vehicle-bridge interaction modeling becomes indispensable for accurately estimating the vehicle's effect on the bridge response. The paper presents expressions for four probability density functions of the chosen metric, facilitating the comparison between the 1-dof and 2-dof models with the concentrated load approach

Gli autori presentano il caso di una bambina di 7 anni affetta da morbo di Crohn con manifestazioni cutanee metastatiche. La peculiarità del quadro genito-anale induceva i curanti a sospettare, inizialmente, un abuso sessuale ed a segnalare subito il caso all’autorità giudiziaria. Successivamente, l’approfondimento clinico ed il quadro istopatologico su biopsie cutanee e della mucosa intestinale consentivano di pervenire alla diagnosi corretta. Prendendo spunto da questa vicenda gli A.A. espongono quindi, alcune considerazioni sul dovere di denuncia all’autorità giudiziaria per i casi di sospetto abuso sessuale sui minori suggerendo l’opportunità di subordinare la segnalazione al raggiungimento di un’adeguata spiegazione eziopatogenetica clinica delle vicende in esame, previa adeguata analisi di ogni utile elemento clinico e circostanziale.
SUMMARY
The authors describe a rare case of metastatic Crohn’s disease presenting as suspected abuse in a 7-year-old girl. According to the clinical features of the perivulvar and perirectal lesions, the physicians transmitted a criminal prosecution. The clinical investigation, as well as skin and intestinal biopsy study allowed to achieve a correct diagnosis. Based on these findings, the authors make some observation about the duty of the report in the case of suspected sex child abuse, and suggest the opportunity to take a careful management in these circumstances
