    Hospital case mix and capacity planning involves the decision making both on patient volumes that can be taken care of at a hospital and on resource requirements and capacity management. In this research, to advance both the hospital resource efficiency and the health care service level, a multilevel integrative approach to the planning problem is proposed on the basis of mathematical programming modeling and simulation analysis. It consists of three stages, namely the case mix planning phase, the master surgery scheduling phase and the operational performance evaluation phase. At the case mix planning phase, a hospital is assumed to choose the optimal patient mix and volume that can bring the maximum overall financial contribution under the given resource capacity. Then, in order to improve the patient service level potentially, the total expected bed shortage due to the variable length of stay of patients is minimized through reallocating the bed capacity and building balanced master surgery schedules at the master surgery scheduling phase. After that, the performance evaluation is carried out at the operational stage through simulation analysis, and a few effective operational policies are suggested and analyzed to enhance the trade-offs between resource efficiency and service level.     The profession of nursing has undergone significant changes in Ireland in the last decade, within the wider health service reform context. Determining the optimal nursing skill mix for acute hospitals is perceived as important to ensure the efficient use of the nursing workforce in the provision of safe and high quality care. However, the term nursing skill mix, though widely used, is contested, and practices to determine skill mix in acute hospitals in Ireland are poorly understood. The aim of the study was to examine contemporary policy and practice in determining optimal nursing skill mix in acute hospitals in Ireland. The objectives were to analyse current policy approaches used to determine skill mix nationally and internationally, to examine how the term nursing skill mix is understood and used by managers and policy makers in Ireland and to explore nursing roles in contemporary practice. A review of national and international policy was undertaken highlighting that despite increasing interest nationally and internationally in various approaches to determining optimal nursing skill mix that there is little evidence base for or standardisation of approaches. Following this, semi-structured interviews with 54 nursing managers and policy makers were conducted to explore different perspectives on how nursing skill mix is understood and determined in acute hospitals in Ireland. Themes identified in this study relate to the diversity of influences on skill mix; nursing role ambiguities and inconsistencies in nursing skill mix practices. Results from the quantitative analysis of closed-ended questions confirmed a lack of clarity of who is involved in determining nursing skill mix, the various influential factors and the main principles associated with determining nursing skill mix. The key conclusions of this study relate to the subjective-objective paradox between the complexity of everyday clinical practice settings and the objective methods proposed to determine skill mix and the macro-micro disconnect of methods and approaches across policy makers and nurse managers. There is a need to integrate the macro-micro and subjective-objective tensions in the development of skill mix policy and practice for nursing in Ireland. Adopting effective skill mix practices will facilitate the effective implementation of key healthcare reforms that optimally utilise the nursing workforce

    The Effect of Neighbourhood Housing Tenure Mix on Labour Market Outcomes: A Longitudinal Perspective

    This paper investigates the effect of different levels of neighbourhood housing tenure mix on transitions from unemployment to employment and the probability of staying in employment for those with a job. We used individual level data from the Scottish Longitudinal Study (SLS), a 5.3% sample of the Scottish population, covering a 10 year period. We found a strong negative correlation between living in deprived neighbourhoods and labour market outcomes (getting or keeping a job). We found a small, but significant, positive correlation between living in mixed tenure (40-80% social housing) streets and transitions from unemployment to employment.     Asphalt pavement recycling has grown dramatically over the last few years as a viable technology to rehabilitate existing asphalt pavements. Iowa's current Cold In-place Recycling (CIR) practice utilizes a generic recipe specification to define the characteristics of the CIR mixture. As CIR continues to evolve, the desire to place CIR mixture with specific engineering properties requires the use of a mix design process. A new mix design procedure was developed for Cold In-place Recycling using foamed asphalt (CIR-foam) in consideration of its predicted field performance. The new laboratory mix design process was validated against various Reclaimed Asphalt Pavement (RAP) materials to determine its consistency over a wide range of RAP materials available throughout Iowa. The performance tests, which include dynamic modulus test, dynamic creep test and raveling test, were conducted to evaluate the consistency of a new CIR-foam mix design process to ensure reliable mixture performance over a wide range of traffic and climatic conditions. The “lab designed” CIR will allow the pavement designer to take the properties of the CIR into account when determining the overlay thickness

    Paper presented at the 3rd National Symposium on Bridge and Infrastructure Research in Ireland, Dublin, 2006 and at ICAP (International Conference on Asphalt Pavements) conference Quebec, Canada, 2006This paper presents the results of ongoing research on the mechanical performance of a 20mm binder course asphalt pavement mix incorporating recycled asphalt pavement (RAP). A series of binder course mixes were designed containing varying percentages of RAP. A mix made only from virgin material was selected as the control mix for the investigation. The effect of introducing RAP into the binder course mix was evaluated through a series of laboratory tests including the Marshall Test, Indirect Tensile Stiffness Modulus Test, Indirect Tensile Fatigue Test and Water Sensitivity Test. A Circular Wheel Track (CWT) was developed in order to study the dynamic effects of a rolling wheel travelling over an asphalt pavement. The CWT was commissioned within a temperature controlled room along with a customised Data Acquisition (DAQ) system. The system involves the testing of rectangular slabs and allows for the investigation of the dynamic tensile strain. The dynamic tensile strain is digitally recorded and the loading effect assessed. The laboratory tests have shown that the introduction of RAP to the binder course mix resulted in an improvement in all mechanical properties.     The Fleet Size and Mix vehicle Routing Problem with backhauls: Formulation and Set partitioning-based heuristics

    In this paper we present a new variant of the classical Vehicle Routing Problem – the Fleet Size and Mix Vehicle Routing Problem with Backhauls (FSMVRPB). An ILP formulation of the FSMVRPB is presented. Optimal solutions for small size instances are produced and upper and lower bounds are generated for larger ones. In this paper we also propose a Set Partitioning Problem (SPP) based heuristic. Three frameworks are developed and tested on a set of new FSMVRPB data instances which we generated. Computational results are presented which can be used for future benchmarking
