A dermatitis flare after patch testing with Dermatophagoides pteronyssinus in an atopic patient
Chronic actinic dermatitis treated with cyclosporine-A
Chronic actinic dermatitis (CAD) groups together all chronic photodermatosis with light photosensitivity. We report the case of a 69-year-old man who, for over one year, had presented a reddish-brown erythema and shedding, with thickened and hypo-elastic skin on the face, scalp, neck and on the back of the hands and forearms. Patch tests were positive to isoeugenolo 1% and photo-patch tests showed a positivity to phenotiazine 2%. After a short and ineffective treatment with beta-carotene and photo-protectors, the clinical picture was resolved with the administration of oral steroids but with a relapse of the dermatitis once the dosage was lowered. We therefore started treatment with cyclosporine-A (4.5 mg/kg/die), which resulted in a rapid improvement of the clinical picture, but with a reappearance of the manifestations when the dosage was lowered. The treatment was resumed and we observed that the dose of 1.5/mg/kg/die resulted morbus-static. The patient is still being treated with this drug at this dosage. The result is that the disease is under control and no side effects are present. As we consider CAD an invalidating disease it seems to us that Cy-A could be taken into consideration as an alternative to traditional treatments
Dermatite seborroica del volto trattata con una crema a base di furalglucitolo
Seborrheic dermatitis is a very common chronic dermatitis occurring in the areas of the skin rich in sebaceous glands. The etiopathogenetic causes are not definitively known, the refore treatment is only symptomatic and not resolutive. The objective of the present study was to investigate the efficacy and the tolerance of a cream containing furalglucitole, 18-beta- glycyrrhetic acid and allantoin in the treatment of seborrheic dermatitis of the face. Sixty patients HIV-negative, 41 males and 29 females, between 15 and 68 years old, with seborrheic dermatitis of the face were included in the study. The cream was used once a day for three months. Nine patients dropped out. Two patients had a slight improvement, while 49 subjects had a prompt remission of the dermatitis. The cream was well tolerated and no side effects were observed. These results suggest that this cream may be an alternative treatment of seborrheic dermatitis of the face
Carcinosarcoma arising in a patient with multiple cylindromas
Familial cylindromatosis (Brooke-Spiegler syndrome) is a rare autosomal dominant inherited disease characterized by the development of adnexal tumors, mostly cylindromas, but also trichoepitheliomas and spiradenomas. Malignant tumors may occur, usually with the features of a cylindrocarcinoma. The authors describe the case of a 75-year-old woman with the Brooke-Spiegler syndrome who presented with multiple nodules of the scalp, face, and trunk. In 1997 she underwent surgical excision of the entire forehead and scalp with skin grafting. Histologic examination revealed multiple cylindromas, some with areas of spiradenoma and one with an extensive adenomatous component; some trichoepitheliomas were also evident. In 2002, a nodule of the trunk suddenly increased in size and became painful. The lesion was excised and histologic and immunohistochemical evaluation revealed a malignant cutaneous biphasic tumor extending into the subcutis, consisting of a major portion with the features of an adnexal carcinoma and of a minor one of atypical spindle cells. Biphasic malignant skin tumors are rare and only a limited number have been described, none in association with the Brooke-Spiegler syndrome. The authors discuss the morphogenesis of the folliculosebaceous-apocrine unit from which the tumors in this syndrome derive, and the pivotal role of mesenchymal cells in determining the process. Since the Brooke-Spiegler syndrome is characterized by a germline mutation in the CYLD oncosuppressor gene, a biphasic tumor in this setting may represent a true carcinosarcoma
