In literature there are no studies regarding the quality evaluation of the whole triage process; there is a strong need for an appropriate tool, since most systems consider patients’ satisfaction and triage time only. We created a new measurement grid, with several indicators, aimed at evaluating many other aspects of the triage process. Every indicator is associated with a score, and the total score defines the quality level of the nursing triage process. This new system has been tested successfully in a major Milan hospital in 2009, resulting in a peer-reviewed publication.Conclusions: we think this could be an important tool in the Italian ERs. Right now, it seems the only one available to assess the whole triage process

No full textLe infezioni ospedaliere (I.O.) costituiscono un rilevante problema per la sanità pubblica attuale, rappresentando, al loro verificarsi, eventi gravati da conseguenze anche serie sullo stato di salute del paziente e, secondariamente, fonte di ulteriore dispendio di risorse da parte del Servizio Sanitario Nazionale. Scopo del presente lavoro è quello di valutare il ruolo svolto dall’infermiere nella formulazione ed applicazione dei protocolli assistenziali, dal momento che uno dei principali fattori associati al rischio di insorgenza di I.O. è rappresentato dalla carenza nell’applicazione di procedure corrette all’interno delle strutture sanitarie. In questo senso, l’utilizzo di protocolli infermieristici e/o multidisciplinari consentirebbe di diminuire la variabilità di alcune attività assistenziali e di proporre una pratica basata sulle evidenze scientifiche, dovere, quest’ultimo, nel caso dell’infermiere, deontologicamente sancito. In relazione al contenimento dei fattori associati al rischio di I.O., anche attraverso lo strumento del protocollo, si ravvisano tre possibili livelli di responsabilità infermieristica propria di specifici ruoli (Infermiere Incaricato del Controllo delle Infezioni; Infermiere Dirigente; Infermiere operante nella pratica clinica) ai quali vengono attribuite peculiari competenze (core competences). Tuttavia la standardizzazione dei comportamenti, seppur di assoluta significatività, non risulta sufficiente, al fine di perseguire l’obiettivo sopraddetto, dal momento che sul verificarsi di tali eventi incidono in modo rilevante elementi economici, logistici e di risorse umane, quali l’attuale carenza numerica infermieristica

In this units there is a strong integration between the different figures of the staff; although that there is no analysis focused on the components that should play a role in the determination of the organic, especially of the nursing staff. First of all you have to choose the staff looking to the complexity of the surgery. Integrating three different systems of classification validated and adopted by nurses, anaesthetists and surgeons, you will be able to define surgery to be a low, medium or high risk. In relation to the clinical complexity and the nursing, care must also provided for a specific location of the patient in operating theatre and define the absorption of resources for each type of operation through the use of flow-chart. The assessment of the expertise is fundamental to allocate human resources based on the operating theatre activity. You can get a classification of nursing staff according to the gained skills. Finally the organic budget must be the result of calculations arising from a specific methodology in which it relates productivity, understood as hours of operation of the theatre and the various related services such as recovery-room, room urgencies, etc., with the individual time of work (TLI) of the professional figure of reference. The Integration of these factors is the key for the determination of the nursing needs of the modern operators section
