The 'Caring' Approach: A Theoretical Basis for Comprehensive Sexuality Education
The paper presents a theoretical and empirical research to frame Sexuality Education (SE) within an
innovative reference: the Theory of Care.
SE is recognized by the world's leading health and educational institutions necessary to promote the health
and full development of young people. The most recent research recommends a model called
Comprehensive Sexuality Education (CSE), a curriculum-based approach that goes beyond a strictly
health risk prevention vision to include the cognitive, emotional, physical and social aspects of sexuality.
In 2024, Italy is one of the very few European countries that have not yet made SE mandatory in schools:
current initiatives are fragmented, while Italian students, who understand the value of SE, consider the
context school to be most appropriate. In response to this gap, the EduForIST project “Development of
technical-practical tools for carrying out educational and training activities in the field of sexuality,
relationships and prevention of STIs (Sexually Transmitted Infections) in the school setting” was funded
from 2019 to 2025. This interdisciplinary project is carried out at the national level, with financial support
from the Ministry of Health. The project adopted the CSE perspective; however, it recognized that it lacked
a radical theoretical foundation. SE is first and foremost a pedagogical action: therefore, it requires a
pedagogical wisdom capable of grounding actions that should not be mere informative or preventive
interventions, but real opportunities for the formation of the person, including in the dimension of being that
is their sexuality.
The EduForIST project has found in the philosophy of care the theoretical frame of reference to ground
CSE on a rigorous basis. The proposal presents a phenomenology of human being and their sexuality
and a phenomenology of the educative act, conceived as an act of care. In Aristotelian terms, care is
the practice that aims at the flourishing of the human being in all its aspects: consequently, a "caring
CSE" can be understood as an act of care that contributes to the full flourishing of people, including in
its sexual dimension. The importance of the theoretical model is confirmed by the qualitative empirical
research carried out in the project. Indeed, the educators who implemented the activities were asked to
write a daily reflective journal and/or summarize what happened in the classroom and their own
reflections on it through a SWOT analysis. As a result, 48 documents were collected that provide insights
for improving the design and implementation. The journals and the SWOT analysis were analyzed using
the empirical phenomenological method, an innovative reading of Husserl applied to empirical research,
since the data consist of a lived experience of the subject, the deep meaning of which can be grasped
through the epochè and the search for the essence.
Through a gradual coding of the texts and the construction of hierarchies of essences, we noted how
educators recognized the "caring" approach of CSE as capable of fostering a meaningful relationship
by meeting the needs and desires of young people and building a constructive relationship with
educational agencies (school, family, communities).

E-mail: [email protected]: La apendicitis aguda es una de las patologías quirúrgicas más frecuentes en pediatría y la apendicectomía un procedimiento históricamente probado para su tratamiento. Sin embargo, se ha llamado la atención sobre la morbilidad y costos que implican los casos de apendicectomías no terapéuticas (ANT). Realizamos un estudio para determinar la proporción de ANT en nuestro hospital, analizar sus características y plantear hipótesis de trabajo que nos permitan disminuir su incidencia.
Métodos: Se realizó un análisis retrospectivo incluyendo todos los pacientes que fueron intervenidos quirúrgicamente desde el 1/1/13 hasta el 31/12/13 con diagnóstico clínico presuntivo de apendicitis aguda, en el Hospital Pediátrico del Centro Hospitalario Pereira Rossell. Se analizaron la edad, el sexo, la vía de abordaje, los hallazgos intra-operatorios, el diagnóstico anátomo-patológico y el uso y los resultados del hemograma y la ecografía.
Resultados: En el período mencionado se realizaron 287 apendicectomías en pacientes con diag-nóstico de apendicitis aguda, de los cuales 146 (50,87%) fueron operados por medio de un abordaje convencional y 141 (49,13%) por abordaje laparoscópico. El índice de ANT fue de 10,1% siendo similares las tasas para ambas vías de abordaje (10,27% convencional vs 9,92% laparoscópica). Por otra parte, se evidenció una mayor incidencia de ANT en el sexo femenino (16,6% vs 5,8%). Respecto a la franja etaria, la tasa de ANT fue mayor entre los pacientes de 11-15 años(12,19%), mientras que para las edades comprendidas entre 6-10 años fue de 8,08% y para los pacientes de 0-5 años fue de 4,16%. De los 29 pacientes que presentaron apéndice sano, 12 presentaban adenitis mesentérica, una paciente presentaba una rotura folicular y una paciente presentaba una peritonitis de causa médica. En 15 pacientes no se encontraron otras alteraciones.
Conclusiones: El índice de apendicectomías no terapéuticas en nuestro hospital fue del 10,10% en el año 2013. La experiencia internacional y algunos trabajos nacionales demuestran que este resultado es mejorable. Se plantea la utilización de algoritmos clínicos para mejorar la presunción diagnóstica y hacer el mejor uso de los recursos diagnósticos disponibles, así como la instauración de un sistema de registro y análisis prospectivo de los resultados clínicos en apendicitis aguda.Objectives: Acute appendicitis is one of the most frequent pediatric surgical diseases and appendec-tomy is a historically proved treatment. However, attention has been directed at morbidity and costs of negative appendectomy (NA). Here we analyze the incidence of NA at our hospital and its characteris-tics in order to propose a working hypothesis that allows a decrease in its incidence.
Methods: A retrospective analysis including all patients who underwent surgery from 01/01/13 to 31/12/13, with presumptive clinical diagnosis of acute appendicitis, in the Pediatric Hospital of the Pereira Rossell Hospital Center, was done. Age, sex, surgical approach, operative findings, pathologic diagnosis and the use and results of laboratory and ultrasound, were analyzed.
Results: In the referred period, appendectomies were performed in 287 patients with diagnosis of acute appendicitis, of which 146 (50.87%) were operated by a conventional approach and 141 (49.13%) by laparoscopic approach. The NA were 10.10%, with similar rates for both approaches (10.27% vs 9.92%, conventionalvslaparoscopic). A higher incidence of of NA was evident in girls, (16,6% vs 5,8% conventional vs laparoscopic).With respect to the age group, the rate of NA was higher among 11-15 years old pa-tients (12,19%), while for those aged between 6-10 years it was 8.08 % and for patients aged 0-5 years it was 4.16%. Of the 29 patients who had a healthy appendix, 12 had mesenteric adenitis, one patient had a follicular rupture and other had a medical peritonitis. In 15 patients no other abnormalities were found.
Conclusions: The NA rate at our Hospital was 10.10% during 2013. The international experience and some national works prove that this result is improvable. We propose the utilization of clinical sco-res in order to improve clinical diagnostic and rational use of ancillary resources and the installation of a prospective registry and analysis system of results in acute appendicitis.Introdução: Oblito é um evento de baixa freqüência, mas que pode gerar uma grave morbidade. Embora alguns fatores de risco tenham sido reconhecidos e desenvolvidos sistemas de detecção com-plementares, continua a ser um grande problema do ponto de vista da saúde e do direito médico legal.Objetivos: Analisar uma casuística do Hospital Maciel, sua apresentação e complicações.
Pacientes e métodos: sete pacientes foram atendidos nos Serviços de Cirurgia Geral e Urologia do Hospital Maciel, no período entre 2013 e 2016. As principais variáveis analisadas são a apresentação e complicações, bem como a presença de fatores risco.Resultados:a descoberta foi de compressas em todos os pacientes. Em 2 casos, o oblito foi assintomático e em 5 casos foi sintomático No segundo, em 2 a apresentação foi temprana e em 3foi tardia. A contagem de materiais estava incorreta em apenas um caso. Nenhum paciente morreu pelas complicações resultantes.
Discussão:o oblito em cirurgia é um problema que continuará a existir enquanto houver pro-cedimentos cirúrgicos, por isso é necessário enfatizar a abordagem sistemática, e também há métodos de diagnóstico que podem ser implementados.
Conclusões: encontramos quase sem exceção a presença de fatores de risco conhecidos em nossa série, embora o número de casos seja pequeno. A incidência também parece ser um pouco maior do que a relatada na literatura

School-based comprehensive sexual health education - from evidence to implementation and evaluation: case study from italy
Brief outline of the overall workshop Sexual and reproductive health (SRH) is a key dimension of health and well-being of individuals, especially relevant for young people. Global health organisations (e.g. WHO, UNESCO, E/CDC) and multicountry reviews recognise school-based sexuality education (SBSE), as one of the most important means of promoting sexual well-being of adolescents and a key component of HIV and sexually transmitted infections (STIs) prevention. International evidence recommends that SBSE should be addressed with a global approach to sexuality, that UNESCO defines as comprehensive sexuality education (CSE). SBSE, however, is still not available for many young people around the world, including Italian adolescents. Italy, in fact, is one of a few European countries that still lacks a comprehensive approach to SBSE coherently and equally implemented across the country, or included in schools’ curricula. EduForIST project, funded by the Italian Ministry of Health in 2019, aims at providing an overview of SBSE in Italy and to develop and implement two pilot projects in secondary schools across the country. Specific aims/objectives and component parts Introduction: Sexuality and education: how to address this dimension in schools?� Sexuality education represents an educative action that, nurtured by medical and scientific information, is aimed at making people flourishing in all the directions of their existence. * School-based sexuality education in Europe and central asia: 2016 IPPF and BZgA initiated a scope review to get an overview of the progress made in developing and integrating SBSE into formal school settings in 26 countries of the WHO European Region. The main key findings and recommendations as well as a global overview will be addressed. * Co-creation of sbse interventions in lower and upper secondary schools in Italy: EduForIST is a multidisciplinary and intersectoral project that aims at providing theoretical and practical tools for SBSE to be implemented in Italian schools. * Implementing comprehensive sbse interventions in Italy-experiences from the field: report on the results and evaluation of EduForIST pilot projects in secondary schools. * How to evaluate a sbse intervention: experience from an italian pilot project: starting from an epistemological reflection on the concept of evaluation in education, we proposed a framework that enable sexual educators in evaluating the implementation of the pilot project, using a qualitative approach. The written texts have been analysed through a phenomenological approach, showing the strengths and weaknesses of the project, the favourable conditions of the setting/ institutions and the postures requested to a competent educator. Conclusions: Future perspectives and recommendations for sbse in Italy Key questions The workshop aims at enhancing public health interest and to foster debate on the topic of SBSE and the need to promote its implementation in the school context. The following questions will be addressed:-which role can school play in promoting SRH?-what is CSE and which topics should address?-which stakeholders should be involved in the creation and implementation of SBSE interventions?-which outcomes can be evaluated and which results can be expected?-how can a SBSE intervention be evaluated
Co-creation of a school-based sexual health education intervention in Italy: Alice Chinelli
INTRODUCTION: School-based sexuality education (SBSE) is the most effective way to positively impact young people's behaviour and attitude towards sexuality. This study describes the development of a SBSE pilot activity targeting lower secondary schools (LSS) within the context of EduforIST project funded by the Italian Ministry of Health. METHODS: A desk review was carried out to collect information about national policies, international literature and guidelines on SE and STIs prevention. An online survey was developed to collect information on SBSE activities implemented in Italy during 2016-2020. Focus groups among project partners and open consultations with a multisectorial expert advisory board were organised. SBSE pilot activity was developed by an interdisciplinary team of pedagogists, public health and SE experts, educators. RESULTS AND DISCUSSION: The SBSE was structured in: a) 5 interactive interventions of 2 hours each with students (4 theoretical and practical modules; 1 final intervention for students-led discussion and evaluation); b) introductory and closing meetings with teachers and with parents. The modules addressed the following dimensions: changes in adolescence; emotions and relationships; sexual identities and diversity; sexual consent, STIs/pregnancy prevention, sexual health services. Additional materials were developed: a. pre/post evaluation tools for educators and students; b. pre-implementation checklist for schools; c. materials for teachers’ and parents’ engagement before/after the SBSE activity. CONCLUSIONS: This pilot activity represented a first step towards the development of a standardised, evidence-based and comprehensive approach to SBSE, for future implementation across the country
