A perplexing primary novel hilar lesion: Remember the pneumonic PPNHL!
A 60-year-old female presented with dyspnea, cough, and chest pain with a left hilar mass lesion. In our case, clinicoradiological correlation, bronchoscopy, and computed tomography-guided biopsy revealed the diagnosis of primary pulmonary non-Hodgkin's lymphoma (PPNHL) on histopathology and immunohistochemistry. We discuss the approach to hilar masses. PPNHL is a rare malignant lymphoma most common being mucosa-associated lymphoid tissue lymphoma. Various therapeutic options are available. Transbronchial lung biopsy in interstitial lung diseases at a tertiary care center in Western India
Background: Interstitial lung diseases (ILDs) are a heterogeneous group of disorders with a wide spectrum. Diagnosis of ILD is by a multidisciplinary approach with clinical, radiological, and histopathological correlation. With the increasing knowledge on the clinical and radiological spectrum, there is a renewed interest to clarify and research the pathological aspect too. Materials and Methods: A retrospective study was conducted at a tertiary care hospital based on the available medical records of 65 patients with IEC permission. The role of transbronchial lung biopsy (TBLB) in diagnosing ILD was studied by comparing with high-resolution computed tomography findings. Data were analyzed in percentages. Results and Interpretation: Out of 65 cases, there were 30 cases (46%) of chronic hypersensitivity pneumonitis (HP), 12 cases (18%) of sarcoidosis, 12 cases (18%) of usual interstitial pneumonia (UIP), 9 cases (14%) of nonspecific interstitial pneumonia (NSIP), 1 case (2%) of lymphocytic interstitial pneumonia (LIP), and 1 (2%) case of cryptogenic organizing pneumonia (COP). TBLB yielded pathological diagnosis suggesting ILDs in 65% of patients. In addition, ill-formed granulomas were observed in 14 of 30 (47%) of chronic HP, noncaseating granulomas in 7 of 12 (58%) of sarcoidosis, and diffuse lymphocytic infiltrates and organizing pneumonia pattern in 100% of LIP and COP patients, respectively, whereas yield in cases of UIP and NSIP is 17% and 33%, respectively. Conclusion: Thus, TBLB is a promising minimally invasive technique for diagnosing ILD with a total yield of 65%. More specifically, the yield is more in cases of sarcoidosis, chronic HP, and rare ILDs such as LIP and COP, as compared to UIP and NSIP
DESIGN AND ANALYSIS OF FEMORAL NAIL EXTRACTION INSTRUMENT FOR HUMAN FEMUR BONE
Different conventional techniques have been noticed to remove intramedullary femur distal nail, a simple method is explained to remove a broken distal closed section intramedullary nail with nail extraction instrument. The surgical technique is described. This technique reduces physical torture of patient during surgery and headache of doctors. In this article the design procedure for nail extraction instrument is explained as well as analysis results (in ANSYS) of nail extraction instrument are discussed. This technique involves removal of distal broken femur nail by on the spot creating threads in the nail, which is one of the feasible technique & the results of this analysis are helpful for orthopedic surgeons for clinical interest
Development of Aluminium Based Silicon Carbide Particulate Metal Matrix Composite for Spur Gear
AbstractMMCs are made by dispersing a reinforcing material into a metal matrix. They are prepared by powder metallurgy and casting, although several technical challenges exist with casting technology. Achieving a homogeneous distribution of reinforcement within the matrix is one such challenge, and this affects directly on the properties and quality of composite.The aluminum alloy composite materials consist of high strength, high stiffness, more thermal stability, more corrosion and wear resistance, and more fatigue life. aluminum alloy materials found to be the best alternative with its unique capacity of designing the materials to give required properties.In this work a composite is developed by adding silicon carbide in Aluminum metal by mass ratio 2.5%, 5%, 7.5% and 10%. The composite is prepared by stir casting technique. Mechanical tests such as hardness test, microstructure test are conducted. It is proposed to use this material for power transmitting elements such as gears which are subjected to continuous loading. Finally modeling and finite element analysis of gear is done using CATIA and ANSYS 14.0.In case of increased silicon carbide content, the hardness, and material toughness are enhanced. From the results it is concluded that composite material such as aluminum silicon carbide is one of the option as a material for power transmission gears
Puzzling encounter of idiopathic pasteurella bacteraemia
is a gram-negative coccobacillus that is commonly transmitted through animal bites including cats and dogs. The degree of infection can be worrisome in the immunosuppressed population with a stark correlation in patients with cirrhosis. However, taking that population into account, only 13 cases of bacteraemia have been recorded with the majority of those cases having cirrhotic liver disease along with multiple comorbidities. Here, we present an elderly patient with only pertinent medical history of mixed hyperlipidaemia who presents after a mechanical fall with acute renal failure and septic shock secondary to bacteraemia
STUDY OF EFFECT OF SOLID CONTAMINANTS IN THE LUBRICANT ON BALL BEARINGS VIBRATION
In deep groove ball bearings contamination of lubricant grease by solid particles is one of the main reason for early bearing failure. To deal with such problem, it is fundamental not only the use of reliable techniques concerning detection of solid contamination but also the investigation of the effects of certain contaminant characteristics on bearing performance. Nowadays the techniques such as vibration measurements are being increasingly used for on-time monitoring of machinery performance. The present work investigates the effect of lubricant contamination by solid particles on the dynamic behavior of rolling bearings, in order to determine the trends in the amounts of vibration affected by contamination in the Grease and by the bearing wear itself. Experimental tests are performed with Deep-groove ball bearings. The Dolomite powder in three concentration levels and different particle sizes was used to contaminate the grease. Vibration signals were analyzed in terms of Root Mean Square (RMS) values and also in terms of defect frequencies
Allergic bronchopulmonary aspergillosis masquerading as malignancy in a nonasthmatic: A rare case report
Allergic bronchopulmonary aspergillosis (ABPA) is a lung disease caused by hypersensitivity reaction to antigens of Aspergillus species of which Aspergillus fumigatus is the most common culprit. ABPA is most commonly diagnosed in patients with a history of uncontrolled bronchial asthma despite optimum treatment. ABPA is rarely diagnosed in nonasthmatics. Diverse radiological appearances are known in ABPA patients; however, whole lung collapse has been rarely reported. We report a case of ABPA in a 62-year-old female with no background history of asthma, initially suspected to have bronchogenic carcinoma of the right lung. After ruling out malignancy, the diagnosis of ABPA was established based on Rosenberg-Patterson criteria. Treatment with oral steroids for 6 months in tapering doses lead to clinical and radiological improvement and decrease in IgE levels.
