    Sudden Blindness in Children Passing Roundworm Per Oral

    Background: Ascaris lumbricoides infestation is the most prevalent parasitic infection among the children in tropical and developing countries but the incidence of sudden blindness after passing the worm per oral is undocumented. The lag period depends on the prodromes. Investigations reveal mere raised eosinophilic count and decreased hemoglobin with normal CT scan and CSF examination. Materials: Ten cases of sudden blindness investigated and treated at various centers without any positive response attending our center after 30-45 days of incidence from January 2018 to March 2019, were selected. Methods: Selected patients’ parents were interrogated for the course of disease, treatment taken and their response. Patients were clinically examined, investigated for basic bioparameters, vision and were treated with the prescribed regime-containing pyridoxine, methylcobalamin, nicotinamide, pantothenic acid and herbal neurovitalizer composite. Results: All patients had progressive vision gain and attained complete vision after 6 months therapy without any adversity and residual effect or any alteration in hepatorenal profile. Conclusion: Sudden blindness in children after passing roundworm or with history of roundworm must be suspected for photoreceptor blockade by roundworm toxin and be treated with pyridoxine and herbal neurovitalizer to assure complete recovery

    Pancytopenia in Indian Children: A Clinicohematological Analysis

    Objective: To determine the etiological profile of pancytopenia in pediatric patients in India. Material and methods: Medical records review of a 5-year period between 1st September 1997 and 31st August 2002. Clinical and hematological data of all patients with pancytopenia (hemoglobin [Hb] ≤10 g/dL, TLC ≤4 × 109/L, platelet count ≤150 × 109/L) at presentation were analyzed. Patients on cytotoxic chemotherapy, those developing pancytopenia during hospital stay, patients referred from other centers with hematological malignancies and neonates were excluded. Results: Forty-two children (mean age 8.26 years, range 8.5 months to 13 years, M:F : 1:0.8) were included. Megaloblastic anemia, aplastic anemia and infections were commonest causes, being responsible for 25%, 19.6% and 32.1% of the cases, respectively. Bone-marrow aspiration (BMA) was helpful in reaching a definitive diagnosis in 92.8% of those in whom sufficient marrow tissue was retrieved for analysis. Aplastic anemia was the commonest reason for failure of BMA in providing a diagnosis. Conclusions: Majority (almost 60%) of the causes of pancytopenia among pediatric patients in this region are easily treatable. There is a need to be aware of such conditions and appropriate investigative modalities should be undertaken for the same

    A Cross-sectional Study on Prescribing Pattern for Children at Primary Health Care Clinics

    Background: Rational prescribing for children is very essential as there is increased risk from the use of medicines in them due to multiple reasons ranging from altered pharmacokinetics to long-term side effects. Drug-related needs of children must be assessed on individual basis to meet appropriate health care outcomes. Aim: This cross-sectional descriptive study aims at assessing drug use pattern and rationality in prescribing pattern as per World Health Organization (WHO) core prescribing indicators. Material and methods: A cross-sectional and prospective study was carried out in private primary health care clinics of Hyderabad, Telangana State. A total number of 300 prescriptions for children were reviewed. Patients’ demographic characteristics, diagnosis and drugs prescribed were recorded in a prestructured and validated data collection form. Results: Average number of drugs per prescription were 1.92. Fever and upper respiratory tract infections were found to be common complaints in this age group. Paracetamol was the most commonly prescribed medication and among prescribed antibiotics, fluoroquinolones occupied the major part. About 67.3% of drugs were from the WHO Model List of Essential Medicines for Children. The percentage of drugs prescribed with generic names was very less. Conclusion: In this study, it was found that the prescription pattern in the selected primary health care centers in Hyderabad was in compliance with the WHO prescribing indicators, except the generic prescribing practice

    Diagnostic Utility of GeneXpert (CB-NAAT) and BACTEC (960) and Socio-clinical Profile of Children with Tuberculous Meningitis

    Introduction: Tuberculosis has always remained puzzling to diagnose. Many children with tuberculous meningitis are diagnosed late due to non-availability of rapid and sensitive diagnostic tests. This study examines the diagnostic utility of GeneXpert test and BACTEC (960) and compares of the sensitivity and specificity of these newer tests. Material and methods: This hospital-based prospective study was conducted in the Dept. of Pediatrics, Sir Padampat Mother and Child Health Institute, SMS Medical College, Jaipur, Rajasthan. Seventy children of >6 month and <18 years age, presenting with fever of >2 weeks duration, headache, signs of meningeal irritation, altered consciousness level and focal neurological deficits were included in the study. Results: Out of 14 patients detected positive by GeneXpert, BACTEC (960) was detected positive in only 4 patients. Out of 56 negative patients detected by GeneXpert, 2 patients were detected positive by BACTEC (960). The sensitivity was 66.67%, specificity was 84.38%, positive predictive value was 28.57%, negative predictive value was 96.63% and accuracy was 82.86%. Conclusions: The GeneXpert has higher sensitivity compared to other currently available diagnostic modalities including liquid culture BACTEC
