    Analgesia considerations in orthopaedic surgery: the role of magnesium sulfate infusions

    Orthopaedic surgical operations are associated with significant post-operative pain, often managed with opioids, which carry risks of adverse effects and dependency. Magnesium sulfate, a NMDA receptor antagonist with analgesic and muscle relaxant properties, has emerged as a potential adjunct to improve pain control and reduce opioid consumption in orthopaedic procedures. Current evidence supports magnesium sulfate as a valuable adjunct in orthopaedic pain management, particularly in reducing opioid consumption and enhancing muscle relaxation. However, heterogeneity in study design, administration protocols, and patient populations warrants cautious interpretation. Monitoring for side effects such as hypotension and respiratory depression remains essential. Further high-quality, standardized trials are needed to optimize dosing strategies and confirm long-term benefits

    Posteromedial varus fatigue fragment (PVFF) in severe varus knee osteoarthritis phenotype: incidence, surgical implications, and management

    Purpose: Severe varus knee osteoarthritis (OA) alters weight-bearing mechanics, leading to progressive stress concentration on the posteromedial tibial plateau. In select cases, this results in the development of a Posteromedial Varus Fatigue Fragment (PVFF), a chronic stress-related fracture that remains ununited and influences knee stability, surgical planning, and implant selection. This study aims to evaluate the incidence, radiographic detectability, and intraoperative significance of PVFF in patients undergoing total knee arthroplasty (TKA). Methods: A retrospective analysis was conducted of 856 consecutive TKA cases performed by a single surgeon. Preoperative radiographs, intraoperative findings, and surgical modifications were assessed to determine the incidence and implications of PVFF. Correlation with varus severity and absence of ACL was done. Results: PVFF was detected intraoperatively in 17 of 856 cases (1.99%), but only 9 (53%) were visible on pre-op imaging.” All PVFF cases exhibited varus alignment exceeding 15° and complete ACL deficiency. Intraoperatively, fragment removal resulted in an increased medial flexion gap, impacting gap balancing and necessitating adjustments in implant selection, including the use of tibial stems or augments in select cases. Conclusion: PVFF is an underrecognized structural lesion for precision in severe varus knee OA, affecting tibial fixation, load distribution, and medial knee stability. Its presence requires careful intraoperative assessment, as fragment removal can alter gap balancing. Improved preoperative recognition and surgical planning are essential to optimize TKA outcomes in patients. Further prospective studies and biomechanical analyses are needed to better understand PVFF’s long-term clinical implications and refine surgical strategies

    An increased incidence of avascular necrosis as the predisposing aetiology for primary total hip arthroplasty in sub-Saharan Africa – a retrospective review of 1,400 consecutive patients

    Introduction: Worldwide, more than 1 million Total Hip Arthroplasties (THAs) are performed annually, with this number predicted to increase by 37.7% by the year 2060. This places a significant financial burden on the healthcare system, with the average cost of a THA being approximately $40,000. Several factors ultimately contribute to patient outcomes and complications, including surgical approach, surgeon’s experience, patient age, BMI, and most importantly, the preoperative diagnosis. Our paper aimed was to describe the various aetiologies of hip pathologies in patients presenting for primary elective THA to a tertiary academic sub-Saharan African institution. Materials and methods: We retrospectively reviewed 1400 consecutive patients presenting for elective primary THA between January 2015 and December 2021. Patients’ preoperative clinical notes, radiological records, and intraoperative results were independently assessed by two examiners to diagnose the hip pathology. A comparison of the presenting preoperative aetiologies was made between those seen in developed countries and those seen in more developing countries. Results: 2176 pathological hips were evaluated. Bilateral pathology was present in 56% of patients, of which 92% had the same pathology. There were 427 (31%) males and 973 (69%) females, with an average patient age of 58 ± 14.13 years and an average BMI of 31.01 ± 15.13 kg/m2. The preoperative aetiologies included primary osteoarthritis (OA) (n = 406 [29%]) and avascular necrosis of the femoral head (AVN) (n = 322 [23%]), of which (n = 162 (58%) had bilateral pathology. The primary cause of AVN was HIV (49%). Patients presenting with AVN were significantly younger (p < 0.0001) and had a lower BMI (p < 0.0001) in comparison to patients presenting for other pathologies. Conclusion: This study underscores the significance of aetiology in THA outcomes and highlights the unique challenges faced in developing countries. By identifying the specific causes of hip pathology in this population, healthcare providers can better allocate resources and develop tailored treatment strategies to improve outcomes in resource-limited settings

    Background: Hinge fracture on the lateral part of the tibia (LHF) is a common complication of medial Open Wedge High Tibial Osteotomy (OWHTO). Many factors have been described as risks for these fractures, but no study has compared an osteotome or an oscillating saw to prevent LHF following OWHTO. Methods: This “propensity-score-matched” (PSM) study was conducted from data obtained in the literature from 1974 to November 2024. A total of 10,368 knees with OWHTO were identified. After 1:1 matching based on correction amount, posterior slope change, surgeon’s experience, the osteotome and oscillating groups comprised 2760 knees each. Results: Among the 5520 knees of the PSM population, the prevalence of LHF was 6.1% in the osteotome alone group (168 cases), and 22% in the oscillating saw group (607 cases). The osteotome group had a significant lower prevalence of hinge fracture than the oscillating saw group (OR, 0.23; 95% CI, 0.19 to 0.27; p < 0.0001) and a lower rate of clinically relevant hinge fractures with revision (OR, 0.34; 95% CI, 0.25 to 0.45; p < 0.001. Discussion: The osteotome may be an appropriate method for preventing hinge fractures following OWHTO

    Limited accuracy of transtibial aiming for anatomical femoral tunnel positioning in ACL reconstruction

    Background: Anterior cruciate ligament (ACL) rupture is a common knee injury, and with advancements in knee arthroscopy, ACL reconstruction has become common. Techniques like single-double bundle and femoral tunnel drilling via transtibial or anteromedial portal approaches are available. This study evaluates the accuracy of femoral tunnel placement via these approaches in single-bundle ACL reconstruction. Materials and methods: Forty-three ACL reconstructions using hamstring grafts were analyzed. Initially, femoral tunnels were drilled via the anteromedial portal from 09:30 to 10:00 (14:00 to 14:30 for left knees). Tibial tunnels (mean anteroposterior angle: 63.5°, sagittal: 64.2°) were then created with the same diameter, accompanied by radiological documentation. A femoral aiming device was used to place a K-wire at the center of the femoral tunnel, recorded photographically. Tunnel diameters included 7 mm (20 cases), 7.5 mm (11 cases), 8 mm (7 cases), 8.5 mm (3 cases), and 9 mm (1 case). Two observers evaluated all radiological and photographic data, focusing on the deviation of the transtibial K-wire from the femoral tunnel center. Results: Of 38 evaluated cases, the transtibial K-wire was within the femoral tunnel in 11 cases (28.9%) – 7 cases with 7 mm, 2 cases each with 7.5 mm and 8 mm diameters. In 23 cases (60.5%), the K-wire was at the perimeter or outside the femoral tunnel – 11 cases with 7 mm, 8 with 7.5 mm, 4 with 8 mm, 3 with 8.5 mm, and 1 with 9 mm diameters. Conclusion: Transtibial aiming for anatomical femoral tunnel positioning is challenging. No significant correlation was found between the transtibial deviation and the tibial tunnel diameter

    Functional positioning in robotic patello-femoral arthroplasty: a step-by-step technique

    Patello-femoral arthroplasty (PFA) is an effective treatment option for isolated patello-femoral osteoarthritis. However, challenges remain regarding implant positioning and patellar tracking. Recent advances in implant design and robotic-assisted techniques have contributed to more personalized and reproducible procedures. Functional positioning (FP), a three-dimensional alignment concept, introduces a customized approach to optimize trochlear resurfacing and restore joint kinematics of the anterior compartment. This article presents a step-by-step surgical technique for PFA using FP principles in combination with an image-based robotic system. The technique ensures accurate preoperative planning, real-time intraoperative adjustments, and precise component placement. The key steps of this surgical technique include trochlear resurfacing assisted by an image-based robotic system and the restoration of patellar tracking, following a step-by-step approach that is both effective and reproducible. The use of FP enables personalized anterior compartment restoration, avoiding overstuffing and improving patellar tracking. Future studies will help refine FP strategies and further optimize outcomes in these patients

    Total knee arthroplasty after anterior cruciate ligament reconstruction with the use of image-based robotic technology and functional alignment

    Background: Total knee arthroplasty (TKA) in patients with prior anterior cruciate ligament reconstruction (ACLR) presents unique challenges due to altered knee kinematics, residual instability, and fixation implants that may interfere with implant positioning. Image-based robotic-assisted TKA enables the functional alignment (FA) strategy that accounts for individual bony anatomy, ligamentous laxities, and anterior compartment characteristics. Surgical technique: This technique involves a CT-based robotic workflow where femoral and tibial components are planned based on patient-specific alignment and soft tissue balance. Intraoperative assessment with a digital tensioning device guides fine-tuning of flexion and extension gaps, ensuring balanced implant positioning while minimizing soft tissue releases. Fixation implants from prior ACLR are identified using robotic navigation, allowing for targeted adjustments such as selective removal or controlled elevation of components to avoid excessive bone loss. Patellar tracking is dynamically evaluated with a probe, facilitating real-time adjustments to optimize mediolateral tracking and anterior offset. Discussion: Given the altered biomechanics in post-ACLR knees, FA may provide a physiological alignment by accommodating native laxities and reducing the risk of residual instability. Additionally, robotic guidance allows for precise management of fixation implants, ensuring optimal implant positioning and bone preservation. While further studies are needed, robotic-assisted FA represents a promising approach for enhancing outcomes in TKA for post-ACLR patients

    Sensitivity of MRI reports for ligamentous injuries in high-grade knee dislocations: A single-center retrospective analysis of radiology reports and operative findings

    Introduction: Knee dislocations, particularly high-grade injuries such as Schenck class KDIV, are complex injuries often resulting from high-energy trauma. While magnetic resonance imaging (MRI) is widely used preoperatively to assess ligamentous damage, its diagnostic accuracy remains uncertain. Methods: A retrospective review was conducted on 92 patients who underwent surgery for a knee dislocation at a Level I trauma center over 10 years. Patients who had a preoperative MRI report and intraoperative confirmation of a KDIV injury without a tibial plateau fracture were included, which left 31 patients. MRI sensitivity was determined by comparing radiology reports to operative findings with fluoroscopic examination under anesthesia (EUA) for injuries to the anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), medial collateral ligament (MCL), lateral collateral ligament (LCL), and posterolateral corner (PLC). Postoperative follow-up documents were reviewed for functional outcomes. A one-way analysis of variance (ANOVA) was performed to evaluate differences in sensitivity across ligament types, followed by a Tukey post hoc test for pairwise comparisons. Mean flexion ROM at final follow-up (≥6 months) was compared between the accurate and inaccurate MRI cohorts using an independent t-test. Results: Only 35.5% of MRI reports fully matched operative findings. MRI sensitivity was 71.0% for the ACL (22/31), 61.3% for the PCL (19/31), 93.5% for the MCL (29/31), 64.5% for the LCL (20/31), and 51.6% for the PLC (16/31). ANOVA revealed that MCL sensitivity was significantly higher than that of the PLC, PCL, and LCL. The difference in mean flexion ROM at final follow-up between accurate and inaccurate MRI cohorts was not statistically significant (p = 0.56). Discussion: Preoperative MRI radiology reports demonstrated substantial limitations in accurately identifying ligamentous injuries in KDIV knee dislocations, particularly involving the PLC, PCL, and LCL. These findings highlight a gap between radiologic interpretation and surgical findings. Surgeons should interpret MRI reports with caution and incorporate fluoroscopic EUA at the time of surgery to ensure a comprehensive assessment of ligamentous damage
